Kids have weird translation abilities with each other. I have a 13yo and 5yo twins. When they were 1, they had no English vocabulary, only twin speak (a made up babble language that the twins use to communicate with each other. This twin language will sometimes carry over into adulthood), and my oldest could understand most of what they were saying. Also, it’s not uncommon for family members to understand a baby and have conversations with the baby long before they develop a full vocabulary, although most of these conversations aren’t as elaborate as Stewie’s conversations.

So I guess no one could understand him until now. Except for Brian. And sometimes Chris. And Meg. And some minor characters. And Peter. And Lois. And.... this doesn't work. If no one had been able to understand Stewie all of this time except for Brian, this might have worked better. I mean, when Susie was finally born, all of her thought were in her head, (voiced by Patrick Stewart), but Stewie has been talking since day one.
