I have seen quite a few videos on this test, conducted professionals who seem to have significant experience in treatment but they all fail to account for the fact that this test has high sensitivity but low specificity, and there is no consensus on any normative measures to confirm its accuracy. It is refreshing to see all those pointed out, you seem well informed.

Concerning, But as long as you don’t have pain in your buttocks or your legs You’re good. Now if I was you, make sure that your not 20 lbs + overweight. Walk a mile 3 times a week.

Can foraminal stenosis also be a cause for a positive test? Or is this test purely for examining the cause by disc protrusion?

Yes, foraminal stenosis can cause a positive test, but it is rare and anyway it is usually closely related to disc herniation. Stenosis does not arise on its own (without a reason), it is the body's defensive reaction in response to damage / inflammation. Thus, typically Patients with stenosis are Patients who have previously suffered damage to the disc or other structures of the spine. In a nutshell, the process can be described as follows: when you damage a healthy disc and it is accompanied by a lot of pain - and then SLR turns out to be positive. In the perspective of a few months / years - the disc heals, but the place around it is "secured" by degenerative changes - than the spine hurts less, but then SLR has a negligible dianostic value (low sensitivity and low specificity).

I have a doubt, for the test to be taken as positive, the pain must come from the lower back or can it be only from the buttock?

The pain must be sharp and radiating. If this is the case, it is really difficult to say whether it begins in the spine or in the buttock (the pain is so bad that the Patient will not be able precise where it started). In practice, Patients with a positive Laseque test result will almost "jump from pain" on the couch with minimal leg elevation. If we lift the leg and there is no sharp shot of pain, and the Patient feels just a pulling or a little pain in the buttock or spine, then the test is negative.

The test may come out positive on both sides. The only question is whether it was properly executed and interpreted, because such a situation is extremely rare. Pain alone is not a positive result - to be considered positive it must be a really sudden radiating pain shot. In the case of a positive bilateral result, most often we deal with a very serious damage to the intervertebral disc, which puts pressure on both the right and left nerve roots. In my practice, I have encountered such a case only once and it was a Patient who was unable to walk at all (was brought to the hospital with a spine injury in a motorcycle accident). I hope this is just a misinterpretation of the test for you. I wish you good health!
