To use a short implant or graft: take into consideration the following: 1. Quality of bone, 2. Expected loads such as history of parafunction, facial morphology ( brachycephalic vs dolichocephalic). I routinely use short implants to reduce the need for grafting but do with it with caution.

Very very interesting. I've had two opposing premolars extracted (primary teeth) from the maxilla here in Thailand. I'm a 47 year old male. I'm now going through the healing process and considering which type of implant therapy to choose to fill the gaps. I've had my 3D/CT X-rays taken, and I've been told I would/should have 2x sinus augmentations with standard implants fitted (same day) - followed by three to six months healing, then the crowns. I haven't been offered short implants at all. They haven't been mentioned. The dental clinic is part of the SMILE Bangkok MALO group, which I understand to be a reputable clinic. I believe I have at least 6mm of vertical bone available, but I'd need that confirmed. I would be so much happier to avoid (possibly unnecessary) invasive surgery with risk of complications and of course significant extra cost. I think I have a few months available to consider treatment options before any noticeable bone loss begins. What should I do? Ask the clinic if they'll use short implants? Ask other clinics in Thailand?
