Haha idk I’m pretty sure you got the bad ending, because you were in purgatory basically and getting off the train was going into the light and dying, but staying on the train meant you would wake up and be alive.

spoiler: I feel like she got the bad ending lol If the siblings getting off the train means they died and went to heaven, then her getting off the train means the same, so she died from the deep vein thrombosis. I think the good ending was helping them reach heaven and them telling you it's not your time to die so you wake up again.
