Really like your videos, especially this RSI series - I recommend it to all residents rotating through the ICU - I think it's important to clarify that ESRD in and of itself is not a reason to avoid succinylcholine - if you know the patient's potassium level is normal, renal disease should not prevent one from using it - additionally, succinylcholine is supported by Cochrane as the paralytic of choice in RSI - that said, it is accurate that emergency medicine providers tend to prefer rocuronium

Ross, thank you so much for this! I am an emergency physician myself. I don’t know why Emergency physicians are more comfortable with rocuronium. Maybe it is a cultural thing, but when I did my residency, it was the paralytic we used the most. I will add though, that I did give more of a consideration for succinylcholine for patient’s that were bound to go to the neurological ICU and require RSI given it’s shorter duration of action and the neuro ICU’s heavy reliance on neurological exam.

@@chefdevil22 the point of the drug is so it can be used for intubation. Intubation is an Advanced Airway that can be put in if a patient can't maintain their airway. A Endotracheal Tube is inserted through the mouth, the Oropharnx, and the Larynx (Vocal Chords) and into the trachea. A Endotracheal Tube allows for them to maintain airway patency. Once a endotracheal tube is inserted, the Patient should be ventilated by positive pressure ventilations (Bag Valve Mask, Continous Positive Airway Pressure device, or a Flow Restricted Oxygen-Powered Ventilation Device.) The way you can tell a endotracheal tube is working is Proper Chest rise and fall and eve chest rise in fall.

That stuff actually works too well for my particular condition! I have a genetically caused deficiency of the enzyme that metabolizes it. It takes much longer to wear off.

They both do the trick, especially when the patient is severely agitated after an OD and needs to be intubated. ROCK is the bomb and SUCKS..

Succ is best when you have diffucult intubation because of short return to spontaneous breathing when things go south

Did I miss the part where you talk about the patient being previously elasticized or at least deeply sedated or is that just assumed. I know you’re not paralyzing a patient who is alert and aware.
