Think of it like the hurricane category system. It’s not meant to be a direct powerscaling system but to see how much of a threat the curse is to normal society. Cat 5 hurricanes mean max damage. That begins at 157 mph. So a hurricane at 500 mph is still technically cat 5 hurricane. That’s because they both do complete destruction, even if one is much stronger than the path. Special grade just means that they can conquer a nation if left unchecked. The special grade curse here would be able to conquer a nation. So would Sukuna. So they would be both special grade since they both can do that.

I think I finally understand how Sukuna healed Itadori's hand by accident. The cursed spirit was throwing condensed cursed energy at Sukuna, but someone, competent enough, can cancel out cursed energy with positive energy or with reverse cursed energy. But reverse cursed energy can also heal and regenerate. What I think happened was that Sukuna meant to cancel out the cursed energy that the spirit was threw at him with reverse cursed energy but by doing so subsequently and ACCIDENTALLY healed Itadori's hand as well.
