You have to remember. While they are like family, the Winchesters treat Cas like complete shit half of the time. I think he remembers that he cares about them deeply, so he wouldn't hurt them, just scare the living shit out of them, but as for other people, he just truly doesn't care. The power that is inside him at this point makes him just completely willing to kill these people if he saw fit.

It's both. His doubts and arrogance taken to a great extreme by the corrupt nature and power of the Leviathans. Power corrupts, and the Leviathans have shown themselves to also be very manipulative. Castiel always seemed to have a genuinely good nature and good intentions but also a bit of arrogance and a lot of doubt and that got expanded.
