About 2 weeks ago I was in a horrific car accident with my fiance and 1 month old son, my fiance ended up requiring extrication from our SUV, I want to say thank you Sam, your videos gave me the knowledge to recognize my Fiance was bleeding internally and begin working on her before the paramedics and aircrew arrived, please never forget the impact your videos have on the world, from a normal guy who gets to hold his family still...thank you.

I'm 13 too and do exactly the same. I have a complete trauma kit, an IFAK and a haemorrhage control kit. I literally spent all me pocket money savings on this and continue to spend all the pocket money I get on enhancing that kit. People call me paranoid but I can't not do this now.

Thank you for these videos. I'm taking a first aid kit with me to protests, and your videos have been so helpful to dispel myths and wives tales on how to treat common issues. I think the biggest takeaway from all of this, is where my limits are and when I need to get EMS involved. To be clear, I'm not trying to pretend that I'm a suitable alternative for EMS. I just know that sometimes it's hard for them to get into the middle of a protest that has gone awry to grab people who need help. Would you consider doing a video for untrained folks that want to help bridge the gap and get people to safety and proper medical attention?

Given your detailed explanation of how you operate and are now not carrying a firearm in your role I think the Leatherman Surge may suit you better. Hey there, very interesting for me as a European is to hear about how different concepts in the US are. Having dedicaded groups to go in along with a SWAT team but not being SWAT officers themselves is something that was new to me. But seems to work well for you guys. Just two questions: how much liberty do you have, when it comes to choosing your gear? I mean, could you say: I'll switch over to 5.11 gear now? And the second question would be whether all the gear you carry is provided by your ems service or if you purchased some of the stuff with your own money. Be safe and healthy out there! Greetings Jan

I was thinking the same thing in regards to the medical gear, its cool stuff yes, but what happens when it's used. In Ontario Canada, the service you work for gives you the equipment, you don't choose and they replace it, just like how a hospital does their logistics. I like the gear shown, but I have the same questions as you.

Usually in the US it’s pretty free to choose your own stuff. Usually not the plates you wear in your plate carrier because that’s a more important decision that they don’t want you making, but they’ll give you some things you can’t buy on your own (I.e. medications you can’t buy without prescription), as well as tell you what to buy. This does vary across the country but most will give you certain things, tell you what to buy when they let you, and also let you personally purchase certain things as well

As your Belt is Primary over either of your vests (light/heavy),, you should have your radio pouch on your belt. Your radio is the "biggest badest" weapon you can carry, as you can call for help, call for additional supplies, call in information that you see that someone else doesn't, etc. Having the headset cables on your vests isn't a big deal, since you can use the radio with or without the headset. Also, while doing SAR or other "rescues", you may not wear the vests, but will always wear the belt. As always, just something to think about.

Current in the service I have had the same situations when I first started out deploying. Then what I did was made sure the pouch I had I the back lined up with my plate carrier and doesn't stick out any further that it. So that way getting in and out of vehicles didn't become as hassle because it just extended the depth of my back versus just sticking out normally.

Why are you using HyFin instead of SAM chest? I heard hyfin isn't as good as SAM, but it takes less space - is that the reason?

Actually it is the opposite. Laminar valves have been proven more effective than valves seen on the SAM chest seal

@@PrepMedic okay, that's interesting. I've always heard (on CLS course for example) that hyfin isn't as good, and main reason for that is because of the not-so-sticky glue, which is much better on SAM- i think i agree with that. As far as i've seen, people dealing with tactical rescue for the first time, tend to use SAM more effectively, I think it's easier to "aim" valve over the wound than with the Hyfin "triangle". Unfortunately, I see a lot of people teaching CLS with no actual experience in saving lives on the field. Maybe you should make an episode about dressings/chest/hemostatics etc. comparing them to each other (e.g. hyfin-SAM-BCS-improvised). It would be very useful, I would for sure send it to my students who like tactical rescue a lot.

I wouldn’t work for an agency that doesn’t allow you to carry a weapon. I’m a SWAT medic in Florida, what the sheriffs agencies don’t understand is, that a SWAT medic that’s not in the stack is no different than a regular Ems unit staging down the street. In this business seconds can be the difference in life or death, not minutes. They’ve got to get the old ways of doing things out of their heads, I had to go to the same SWAT operator school they did so every stack should have a SWAT medic in it.

So while I agree with a lot of what you said, I don’t think you understand how we/other unsworn teams operate (I have been both armed and unarmed). This team also goes through the same SWAT schools as the team and will stack with them if the team will move farther then 30 seconds away from the bearcat or egress during the mission. Obviously it would be better for everyone involved if we were armed but that’s not what our hospitals risk department thinks and I am at least thankful that we can still be in a position to help our teams.

@@PrepMedic thank you for your response, I’m in a similar situation, I work for the Fire Department and I’m also on the SWAT team, admin on both sides don’t seem to agree with us being able to carry, however since our Governor has implemented Florida House Bill 487 we haven’t had any issues. You may reach out to your Governor and could possibly make some needed changes. I’m sure it was a hard decision for you to leave an agency that allowed you to be armed to go to one that doesn’t.

Hey, does somebody have an idea how you could get your hands on proper medical equipment (in this case a tourniquet) if you aren't living in the US (in this case central/western Europe, Germany), without having to pay way more for the same product. I intended to buy a CAT because I thought thirty bucks where fine, but in the end the price turned out to be $45 US on the official website, that North American rescue tells you to order from if you live in Germany.

But what I really meant was, you always bring the airway and the trauma with a tiny med pouch! but looking back I see that a tac medic would bring less in general, and would have to very carefully choose what that is.

@@akadopeboi: Decisions that are borne out during crisis are, at best, a reflection of the operator's history: Options that worked successfully in previous deployments tend to be reinforced in subsequent events. However, although Murphy's Laws always applies during any/all active operations, a tactical medic's focus must constantly remain on: Personal safety/team safety/public safety. Weight v speed is always a serious concern, but should reflect the team's primary goal: Expedient resolution.

Mike, Rule #1: Opponents never play by the rules. Rule #2: We're the visiting team. Always. Rule #3: See #1. Any gear that better helps perform the job w/o violating standing orders, and keeps us from getting hurt/KIA is a go. Or would you prefer that we wear our hi res vests? And many EMTs/Paramedics/Tacmeds come from military service. Busting ours to save yours.
