*Nice kit!Thanks for sharing.* *I have a pretty similar setup for my car bag.* *I added a small and cheap pulse oxymeter to go with the BP cuff and stethoscope.* *One thing I add to large kits, that I can't fit in small kit, is simply a bottle of mineral water (as big as possible).* *You can treat a lot of things with just water (dehydration, flush wounds including eyes, burns, give meds, treat hypoglicemia with sugar, wash your hands etc).*

*I just have a cheap no-brand name one that I bought for $10 or $20 on Amazon I think.* *They have gotten smaller and cheaper and it's as accurate as the more expensive ones we use in the ambulance.* *It's just a quick way to get a pulse reading, I don't really need it for the oxygen saturation level since I don't have O2 in my kit anyway.*

Hey prepmedic. Good video, really enjoyed it as well as the other two I watched about your gear. Want to say that an army medic trick for a manual suction device is a 20 or 60cc syringe and a npa. Put the npa on the end of the syringe and it makes a suction device good for fluids. Might need tape all depending. Play with it and see what works best for you. It's what I used.

I love using the SWAT-T for junctional bleeds. It allows me to wrap a wound and attack it from multiple directions after packing the wound. I also noticed a few items that would be good to add, and this is just my preference, and headlamp, a couple of glow sticks for marking for LZ's, rooms, etc., you can also bungee a cloth litter to the underside of the pack to help with exfil over a limited range, and also an SPO2 monitor. Especially the SPO2 monitor since you really want to be concerned about hypoxia, as well as hypotension before placing an airway.

Nice set up. I’m about to buy the same nar 4 aid bag. i found out its much cheaper if you sort out your own medical supplies than buying it ready. I didn’t see any celox blood clotting applicator. The powder stuff is great and they have one in a plunger.

A quick observation about the cover mesh; Seems like it would be pretty easy to sew on label tags or loop to the mesh for easier identification. Just a thought

When I was a Combat Medic (in ancient times), I created kits for immediate response to blood volume loss ( severe hemorrhage). I placed a 500ml bag of Lactated Ringer's, infusion set, 2 catheters, small roll of tape, 3" elastic bandage, 2" sponges, povidone wipes, surgical gloves and a "Hand Warmer" in a Zip-loc freezer bag. I could pull out everything I needed in one movement, start the infusion and then move to the next casualty. I would use a Hand Warmer against the bag when the fluid was cold and secured it with the elastic bandage. I used the bag, tearing it open and taping it in place, to keep the site from being contaminated by rain, dust ,dirt, etc.

I needed one of those but I thought they were regular paramedics. I actually "died" for 10 minutes and somebody saved me. The doctor didn't believe then (in 1991) that I was watching them do it; i.e., standing outside my own body, feeling nothing they were doing and watching while they did it. I think it was weird how I knew I'd just died but didn't care at all.

How about a cric kit? I suppose in a law enforcement environment maxillofacial trauma may be less common and evac times tend to be shorter than in a combat zone but I could see some situations where a definitive airway might be required, plus a cric roll isn't all that big. I'm not sure how effective needle crics are (in the army we use the traditional scalpel and ET tube) but in my mind having some kind of secure airway is better than no airway.

Can you go over the Newer Olaes Blast Bandage? I've used the 6 and 4 modular ones and carry a few in my larger vehicle kit. I've thought of a blast bandage or 2, what do you think of them? I can send ya one if you want to try them out. P. S. TY on replying for burn kit review, I look forward to it

I'm curious your reasoning on not having a laryngoscope along with some ETs, as opposed to the LTs. ETs provide better airway protection and the addition of the laryngoscope would make your forceps more useful. maybe there's something I don't get about it because my training is in the hospital and there is a rational for using the LTs over the ETs in a pre-hospital setting? I'm interested. In the hospital I can't recall a time we opted for anything but the ET... I mean with the exception of Crics. also, any thought to adding supplies for a cric to your kit? I'm actually not even sure if paramedics are trained on that... be interested to know. I would think that should be within your skillset.

May I suggest the BDS Tactical medic bag. It's a suped up M9 bag. After using both i think it carries my gear better. Been using it for over 10 yrs now

could you just stow away the thick straps in the zipped location and just attach thinner straps to the clips on the bag? looks like you can definitely mod in some thinner straps onto that bag.

Great that you mentioned nsaids before ops. Our boxing coach stressed only tylenol in the days before fights. Would there be a role for opioids in your kit since they also dont encourage bleeding?

Great video, and thanks for your service. Are you familiar with the STAT Tourniquet? If so, what do you think of them? They're a relative new comer, but the quick deploy of these and ease of use as well as their ability to be used on small children has placed them high on my list of considerations, plus they are very compact.

Nice kit and well organized..might want to think about replacing your king LT for iGels..work well and no worry of cuffs to inflate.. the only addition would be a beefy set of hemostats.. great for sticking though the hole of the IV gag and clipping to your patient.

Quick Question: how long does it take in training to become proficient enough in administering trauma first aid to do more good than harm to the patient? I would like to learn these skills but not sure how big of a time commitment it is. Thanks

Living in a country where both police and ambulance are state owned entities I find the concept of a "law enforcement paramedic" quite interesting. So are these people police officers upskilled as paramedics? Here in Australia while we do a lot of scenarios with police and other agencies, we don't have such paramedics. Whenever we get called to a situation we just wait until the scene is safe. One drug introduced in the last few years for us is droperidol, a fast acting sedative for psychotic aggressive patients.

There's an AAO CPG out. It doesn't specify HARU. I'm not QAS, but what I've heard is that HARU primarily is being trained for TECC, although I suppose any ACP/CCP could be.

The thought process in the Midwest is that it's easier to train an already proficient medic to work with officers than the others way around.

Any reason you opt for King tubes, when standard practice globally tends to lean towards iGels? Seems to be an American thing, and if that's what you use in your day job, fair enough. In terms of what you may be missing... - Chest seals? I can't recall if you keep some in your other kits, but it would seem sensible to me to have some in this pack as well so it was comprehensive, even if somewhat redundant. - I've seen tactical paramedics carry razors, as a way to increase the seal of their chest seals. - Cling wrap, or an ABD size Israeli dressing (as it has a removable occlusive layer) may be beneficial for abdominal wounds and/or evisceration, also burns. - Some kind of light source (again, I can't recall what you have elsewhere on your gear), like a headlamp or chemlights.

Sorry about the chest seals. I watched the video in full early, and came back and watched only most of it a second time before I commented. Regarding the 'trauma wrap', was that packaged with the SAM splint? Is it fabric or occlusive? Apologies, it might just be difference in nomenclature between countries. Overall mate, really enjoy your videos, especially since you make a lot of reference to evidence based medicine which is really refreshing.

@@PrepMedic Okay. That is honestly a lot of money to pay out of your own pocket. But I understand you, safety for the team is important. Where I come from we don't pay for the equipment ourself. We get a pretty big budget per man, to get the things we need for service. Thanks for the answer, and keep it up with the great videos 👍

just about every special forces and army medic says to put the tourniquet on the outside of the bag bleeding out can happen with in seconds putting them in a bag within a bag just dose not seem smart

In my experience, most people walk around at least a liter low on fluid to start with. Your not going to hurt an average size person with a 500/1000ml bolus. Especially during bleeding emergencies.

cliftonsuber actually that is false and recent studies have shown that for every 100 ml of non-blood products you infuse into a trauma patient you actually increase their mortality. NS has no clotting factors, actively fights clot formation, increases acidosis, and has no O2 carrying capability. If it isn’t blood products, it is hurting the patient.

Those in plastics are either there to be used only once and to be kept sterile or to be repacked and be used. So it really depends

😂Yeah, it is pricey but pretty par for the course when it comes to a high end medical pack. First Spear, TSSI, Tactical Medical Solutions all sell their packs for about the same amount.

@@PrepMedic I added a couple Celox-A for hard to reach areas, but in general I've never seen proper packing and pressure not stop a heavy bleed... I think the advantage of hemostatics is in deep wounds in the pelvic area and blast wounds (I.E. multiple smaller wounds, or very large wounds) where pressure over a large area/multiple areas may not be feasible with one person/yourself. Other than that, I'd estimate they are grossly over-valued and misunderstood.

Nice, mine is the similar but with glucometer, epi pens, Nubian and Naloxone. More packing gauze for junctional injuries. Added CATs to the outside molle for rapid access in mass casualty event.
