Stallone was a real life cigarette smoker until he was 30. But he quitted when filming Rocky because he was running out of breath in Rocky's iconic training scenes. Then he decided to smoke cigars to fulfill his nicotine addiction. I think his mouth is used to inhale smoke so he struggles to just puff the whole smoke out due to him being a real cigarette smoker for at least 10 years.

I miss cigarettes but I’m glad I quit. Lost a lot of family members to that smelly nonsense. A fresh lit smoke still smells good but after that it’s pretty terrible once you haven’t done it in a while.

From what I know he used to smoke cigs all the way up to rocky, upon realizing he was out of breath during the production of rocky he quit cigs and started smoking cigars. Although some people may not, pretty much everyone inhales cigs, but cigars on the other hand are never to be inhaled. So yeah when he's smokin cigs in movies he's surely inhaling as he's had his fair share of smoking em, and you can also tell he is due to blowing smoke off his nose
