Hello My transfer was done at 8/11/23 I am 5dp5dt today. I took a urine pregnancy on 4dp5dt it came negative I know it’s too early but I have lower back pain from 3dp5dt and it is still continued. After implantation.. my embryo is stuck and got a positive sign.. but no development and got a miscarriage .. took all the tests ..even karyotyping is done.. no problem at all.. but still something is missing.. could you please suggest me any other ideas ..

The commonest reason for a miscarriage is a genetic abnormality in the fetus, and this is Nature's defense mechanism, to prevent the birth of an abnormal baby. While these defects are often random, they are commoner in older women. This is because these eggs have more genetic abnormalities, because they have "aged" and have genetic defects, which be do not yet have the technology to be able to screen for. The fact you have conceived in the past ( even though you did miscarry) means your chances of having a healthy baby are excellent, so please don't get disheartened !

We don't use these oily progesterone injections . They are very painful ! We use progesterone vaginal pessaries. They are better and more effective

Yes. Please take the following supplements to improve your ovarian reserve. 1. Tab DHEA ( Ovigyn DSR) , 75 mg, once a day 2. Cholecalciferol ( Vit D) , 60000 IU, once a day for 4 days; and then once a week for 8 weeks. This is a powder available in a sachet. Dissolve it in milk or water. There are many brand names 3. Tab Folic acid, 5 mg, 1 tab daily 4. Tab Orofer - XT, 1 cap daily. This is a hematinic 5. Tab Ecosprin, 75 mg, once daily. This is a blood thinner ( low dose aspirin) We would use the Letrozole -Antagon protocol for you which we use for poor ovarian responders. This will help you grow more eggs of better quality. ICSI treatment starts from Day 2 of your cycle. Day 1 = Day of bright red bleeding. If the bleeding starts after 6 pm count the next day as Day 1. Ignore the spotting. On Day 2, you need to do an ultrasound scan to confirm there is no ovarian cyst, after which we start your superovulation . Please find a digital ultrasound scanner for your next scan . They will burn the images as a jpeg file on a CD or flash drive. I want the raw images as jpeg files please - not scanned in copies of the paper printout - too much loss of resolution with this ! We can also do the scan for you at our clinic Tab Letrozole 5 mg daily from Day 2 - Day 6 Inj Gonal-F ( 75 IU FSH ), 4/8 amp ( 300/600 IU ) daily from Day 2. The dose of Ginal-F ( FSH) will depend upon your ovarian morphology and your antral follicle count. From Day 7, you stop the Letrozole and start Inj Cetrorelix/Orgalutron/ Antagon, 0.25 mg daily . This is a GnRH antagonist. The Gonal-F continues. We do the next scan on Day 7, after which you would have to be in Bombay for about 10 days. Your husband is needed on Day 12-Day 14 ( the day of the egg pickup). All the treatment is performed at our clinic, which means you never have to go elsewhere. This is what the daily schedule would look like. Day 1. Day 2. Vaginal ultrasound scan to confirm there is no ovarian cyst. If there is no cyst, we can commence superovulation. If there is a cyst, we aspirate ( puncture) it and continue with the treatment. Tab Letrozole 5 mg daily . Inj Gonal-F ( 75 IU), 8 amp ( 600 IU ) daily Day 3 Tab Letrozole 5 mg daily . Vaginal ultrasound scan to monitor follicular growth The Gonal-F and Cetrolix injections continue until the follicles are mature ( approx Day 12). Then the HCG injection is given, and eggs retrieved 36 hours after this. Embryo transfer is performed 5 days later. After the transfer, luteal phase support is provided with daily Progynova ( estradiol valerate2 mg, 3 tab daily; and Uterogestan ( 200 mg), 6 vaginal suppositories daily. You can travel back 3 days after the embryo transfer. 14 days after the transfer, you need to do a blood test for beta HCG to confirm a pregnancy.
