what do u normally recommend for patients who have normal size prostates and yet have bothersome symptoms? in my case my bladder always feels way fuller than it is and the symptoms have worsened over the last few years (i'm 55 now). cysto didn't show the dreaded medial lobe nor anything out of the ordinary. was already taking finasteride before all this started with the bladder discomfort. it's confusing that a surgery that removes mass from the prostate would help when the prostate is already at a normal size. 2nd question, how do u differentiate between a diagnosis of IC vs blaming the proste?

Thx for your questions. The first thing before proceeding with treatment is to confirm if there is an obstruction. If your flow rate is not conclusive and your cystoscopy is “normal” the next step would be Urodynamics. IC is seen endoscopically by filling the bladder, then emptying and refilling to detect bleeding from the bladder lining. Hope this helps

Any thoughts on the promise of Fexapotide?

Great question gissaus. It’s currently being studied and is not available here in Australia to date. The process of treatment appears to be the same as Rezum in that an injection is given into the prostate which results in resorption of prostate tissue and unobstruction. We will need to see what the data shows.
