I'm fascinated by this - I had cataract surgery with lens implant in one eye about 4 years ago. It was amazing - painless and instant and permanent improvement in vision in that eye. I had expected to have to cope with some sort of artefacts in vision because of the artificial lens but there was nothing at all - my field of view is perfectly clear. Because I'm in the UK this was free, fortunately.

In September of 2021 I had laser-assisted cataract lens replacement surgery performed on both eyes at the same time with Panoptix multi-focal lenses. The outcome has been spectacular. I now have 20/20 distance vision, and 20/15 close up. For many years prior I wore soft contact lenses and needed reading glasses when wearing the soft contacts. If I was wearing my regular glasses, in order to read I would take off my regular glasses to see close up. Now, at 68 years old I see extremely clearly at all focal lengths. As an over-the-road truck driver with doing tons of night driving I was concerned about potential star burst patterns, but found them to be of little to no concern. Depending on the brightness and the angle of lights at night I could sometimes see circular rings around bright lights, but not always. At this point two years later, they rarely show up. I did a lot of KZbin video research before the surgery, watching many videos on both sides of the potential issues and side effects. I would NEVER suggest anyone have elective eye surgery such as LASIK. However, if one needs cataract surgery that’s a different story. For anyone needing cataract surgery, I would suggest doing lots of their own research and know up front to the best of their ability what potential side effect they personally might or might not find bothersome. No longer carrying around glasses and constantly putting them on and taking them off has easily been the greatest benefit. By the way, I was back at work as an over-the-road truck driver 48 hours after surgery.

@stratdoc9 ай бұрын
Panoptix, as with all multi focal lenses, divide the light. This means the ability to see shades of light or contrast is diminished. Of course this is also what your cataract has been doing! If we give you this type of lense with a non multifocal lens in the fellow eye you would be surprised by how less defined your vision might seem. That being said patients like you “sell” this lens as when it fits your lifestyle and visual sensitivity it provides a powerful solution. I prefer extended depth of focus (EDOF) as they do not affect contrast and can generally meet your near needs in good light.
@tomwiles9 ай бұрын
@stratdoc That's why it's great that we have choice. In my case I'm an over-the-road truck driver and do an extreme amount of night driving, yet even with the multifocal lenses I still have fantastic night vision. I'm not attempting to sell anything, but am just relating my own personal outcome. The PanOptix lenses have been one of the best things that I've done for myself in years. I can see how people might feel bitter post surgery that they made a wrong choice whatever that choice was.

Like your explanation. I had multifocal lenses put in and after about a year or more, both lenses were covered with protein deposits which had to be removed with a Yag procedure. The procedure was quick outpatient and it worked. I understand not everyone gets this protein build up but some do. Your vision starts getting clouded again. Not to worry just get the Yag.
@bdonkersgoed10 ай бұрын
I don't think it was protein on your lens, but rather on the your natural capsule that the artificial lens is placed inside. This condition is called "Posterior Capsule Opacification" (aka "Secondary Cataract") and it's pretty common after cataract surgery. The YAG laser essentially blasts a hole into the capsule so that light can get through it again. I'll likely have to get the YAG procedure done as well.

Excellent information in your videos. Today I visited my third Ophthalmologist over the past year in search of a concise diagnosis and explanation of my slowly worsening eyesight. The first two doctors recommended cataract surgery ASAP, but I was not experiencing most of the commonly described symptoms of cataracts, which left me confused. Luckily, before this latest visit I found your web site and learned much more about cataracts so that I could ask the right questions of today's eye doctor. Between your very good videos and the good fortune of finding a doctor who had the patience to explain my situation, I learned that cataract surgery will one day be a necessity, but is not now urgent. Thank you for the detailed info - it really helped! My advice to other patients is to beware of surgery mills (who I suspect recommend cataract surgery to just about anyone who walks through their doors), and ask lots of questions before you submit to surgery!

I was terrified before both surgeries. It's a routine surgery, but nothing is routine when it is your eye that's being cut on and removing and replacing some vital paet of your vision. Very glad that i had it done though. I just had the basic lens replacement that's covered by insurance and Medicare. I am one month post my first eye and 2 days post my second eye surgery and am seeing well enough to drive without glasses, watch TV, etc. It's great to be able to see at night again! I am seeing around 20/25 now, which is a drastic improvement for me. I can't read things up close though.

The problem with multifocals is this: the peripheral portion of this lens is usually blocked by iris under the bright sun light and you have to use reading glasses or sun glasses to read. My daughter has this issue. I also had a cataract removed - first on left eye 10 yrs ago and then I’ve lost IOL in this eye 3 yrs ago in car accident . I had 3 surgeries: first on removal of the old IOL and then implanting a new IOL into the anterior chamber, promptly followed by its removal. After that I’ve completely lost all vision in this eye. 5 yrs ago I had cataract removed on my right eye and now, after 5 years I have IOL in this eye shifted down by about 2 mm. It looks (as much, as I still can see) I am gradually going blind. So, my advice to all of you - don’t rush with your cataract surgery and stretch time as much as you can. Remember it is not such a simple operation, as it looks on this video, and, besides, not all doctors are good doctors, especially, while they are working hard on their own assembly lines to maximise production (mainly money) .

Sometime between 2-3 years my right eye developed a haze. I can see through it, but it makes it difficult to read. I can drive on side streets, but not on highways that require quick reaction. I know I should have gone back to the opthomologist, but I had such a bad experience that I'm afraid of it being worse. I don't even know if they do any type of laser surgery. Now that I know it's an easy procedure, I feel somewhat better about checking into it. I really miss being able to see clearly!!

As a result I am always losing them. It's a challenge. So go for the best lens. Yes they are the most expensive but it is so worth it. I got the "better" lens and regret it. Get the best.

Great and easy to follow overview! IOL technology advanced remarkably in the last 10 years. And now it is possible to have most patients fully satisfied with their postoperative vision. The key despite all technology, is to accurately understand the patients daily life activities, expectations, hobbies etc... Then a surgical plan can be costumised to obtain the best glasses-free vision

Hello, I am a color pencil and pastel artist who creates realism. I am having cataract surgery on both my eyes and have been nearsighted all my life. Although I would prefer to be independent of glasses I am choosing mono focal IOL replacement lenses because of the decreased contrast sensitivity of the premium IOLs. I am having difficulty deciding on which distance to set my Mono focal lenses. I would love to maintain my near sightedness without glasses but not sure if I will actually be able to see super fine details and the print on my pencils even with the Mono focal set to near. If I will still need to wear reading glasses to see those fine details I might as well choose Mono focal IOLs set for distance. If you are able to provide some insight I would be very appreciative.

My first day and second day of Cataract surgery, Eyhance Lens….Complete WHITE OUT. Although, a beautiful pure white light. Took another couple days for objects to come into view. 5th day the wax paper sheen disappeared. I could only read the two lines with three letters a week after… I think maybe instead of 20 Feet distance set, it’s set at 200 feet, lol. Things further away seem clearer. Overall it’s already life changing. I can actually see the green light while driving. See curbs. See street signs. My left eye still has the cataract and it’s interesting to A/B each eye. The old eye is a dingy brown with added shadows on everything. New eye is clear with pure colors and whites. I just wish my Eye could see a little clearer , especially evident while reading the eye chart🙏

Hello Doctor, you have explained all the good stuff of Cataract Surgery, now it's time you tell the truth! I had a good friend and two (2) late wives that went thru Cataract Surgery and all three ended up with problems. Everything from infection, wrong lenses, and other problems. Now it would be nice that you Doctors be honest. For the last four (4) years the VA has been on my case to get the surgery. Another item you should tell people is all the eye drops that must be put in the eye after surgery from two weeks to 30 days. This is great, but for me, I could never put eye drops in my eyes and I have nobody else to put drops in. So, to every eye surgent around, I need surgery, but I will not get it. You people (Maybe Doctors) best be on the lookout, because I will be on the road, and you might be my next target.

Sorry to hear your friend and late wives had poor experiences with cataract surgery. There is a risk to every surgery. Thankfully, in my experience, the vast majority of cases never run into complications and the few that I have had to manage were minor and corrected easily or just took an extra few weeks of healing time. Either way though, there is a risk. At one time, there were doctors who did "dropless" cataract surgery, but the risk for infection and more complications went up. Hopefully, if you ever did decide to get surgery, someone could help you with getting the drops in. Otherwise I did make this video to help people with that kzbin.info/www/bejne/iYKaf4uHmLqio68si=xX_i-4_9g4oGs7iv

Don't worry about it doc, you said just what I thought you would say. I was looking for some honesty about the possible neg. effects. At 74, I look for honesty in people before I do anything drastic, and I think surgery on the eyes is a little drastic. Like a dumb kid back in "68" I fell in line and was trustworthy like a lot of young kids. After I came back, trusting people is not my thing, especially doctors!

I'm so glad that I found this video. My eye doctor has been after me for quite awhile about cataract surgery. Just the thought of someone cutting into my eye scares me to death. I've had soft contacts on and off for a few years, one try with hard lenses about two years but they kept moving. Went back to soft but I just stop wearing them because I don't see any reason to wear them if I don't get better vision. My visit this year 2024 cataract surgery was discussed again ... I just can't wrap my head around that. I am going to try a hard contact again to see if the fit will work better. I did have great clear precise vision with the hard lens. I like the fact that there are a few options out there now. Is the surgery under general anesthesia? I'm 78.

I had Lasik back in 1998. I then had cataract surgery around 2015 in both eyes and chose Crystal Lenses because they offered the ability to focus using your eye muscles. I am seeing better than 20/20 near vision in both eyes, plus close to 20/20 in both eyes at distance. Crystal lenses required quite a bit of eye exercises for many months after surgery to get the best results, which I took seriously. I am quite happy with my vision as you can imagine. One question I have is, what category of those you presented, does Crystal Lenses fall into?
@s.mcgonigal2713 Жыл бұрын
I had Crystallenses put in both eyes in August. One I was set for distance, and one was set for near vision. I can see pretty well across all visual ranges, but I have ghosting and streaks with my right eye when I look at lights at night. I had a different kind of visual glare phenomena in my left eye that was very disturbing, but it seems to have gotten a little better recently. I'm wondering if you have had any haloing, glare, or other strange visual phenomena at night with your Crystalenses. The doctors tell me I will neuro-adapt to these phenomena, but they are significant enough that I really don't believe I will ever be able to ignore or be OK with them. Like you, I am using the vision restoration charts, and my vision is improving slowly, but I didn't realize it was going to be such a long ordeal to be able to see clearly at all ranges. i'm really wondering if I should have opted for another choice. I would love to hear more about your experience, if you would care to say more, please. do you have mono vision are blended vision, ordered both eyes get the same lens? Thanks.

Your videos are so helpful and informative. Thank you for your willingness to share your knowledge and evaluations. An idea for a future video...Can you explore contact lens options for people with very high myopia? I have used monivision successfully for a very long time but now that extra years are making presbyopia more of a problem and with the addition of dry eyes, I am finding that my old lenses are not working as well and there are not many alternative options for people like me. I'm getting frustrated with the need to overcorrect everything! I live in a small community so I do a lot of online exploration trying to find newer, better options that I could ask my doctor to look into.

Glad I found this video. I have been going to this one guy for about 7, 8, years. He is 5 minutes from my house, for the past 2, 3 years I tell him, I kinda see double, and my new glasses have not been right. I decided to try an eye doctor, who found I needed a prism, and after a few weeks of test prisms they gave me a script that should have cleared things up. However, they did not take my eyeglass insurance, so I took the script to the guy next to me. (This was last year) when I received my new glasses Nov 2022 they did not seem right, the guy said it may take a few days to get uses to them. Well this Nov, I went back to the eye doctor and when they checked my glasses, they said it was not the script they gave me, no prism, and straight was wrong, and I was lucky to see/read anything. The guy near me put in some BS script and when he checked them, he said they were right. Needless to say, I nor my wife use him anymore, and I tell everyone. However, the Cataract is an entirely different story, the sergeant is trying to dig deep $$$$. If you have time, I can comment on it.

When I have my lenses fitted I plan to have a mono lens for far vision as I think things become over-complicated if they try to correct my astigmatism, myopia and presbyopia at the same time. If my astigmatism changes it will be far easier to change spectacles than to adjust an IOL.

Good info, wish it had been available when I had my surgery at a county hospital. Having uncontrollable Glaucoma, the doctor recommended cataract surgery as a last resort. He promised I'd be seeing better afterward, so I opted for it. It changed my vision from 20/25 without glasses to 20/70 and needing to rely on glasses. It improved my near sight, but I was never before bothered by having to wear 99c glasses to read. The glasses I have to buy now are expensive, and I'm struggling to see through them to drive with all the scratches. Apparently, the surgery didn't eliminate my lens. It seems to have given me a cataract I didn't have before.The doctor told me he just pushed the lens out into the eye, and eventually it would sink down out of view and not be a problem. During the surgery I woke up and looked at the doctor's hands and instruments, aware of having intense pain in the eye. Post surgery the pain was unbearable, and the doctor told me I shouldn't be having any pain, that he had done a great job installing the implant, and he prescribed me some Tylenol. In a few hours I was sent home still in pain. When I went back for follow up, I told the doctor I was not happy with my vision, I felt blind with the new lens. He said the surgery had successfully lowered the pressure in my eye, which was the whole point to doing it in the first place. He said the lens they used was the only lens the county provides, and everyone who gets cataract surgery at that hospital gets that lens. Wow, I thought, to save my sight, the doctor destroyed my vision while promising me better vision. I told him I wanted my old lens back, and he said I'd have to get financial approval for that. While I waited for approval, the county terminated my financial aid and kicked me out of their system. It has been about 17 years since then, my eye has floaters, and my glasses are so scratched I can drive only when the sun is shining brightly. In order to get new glasses, I have to have another optometric eval, and the doctor doesn't accept Medicare, my only insurance now. It seems strange that with a lens implant, I'd still need a vision check. Wouldn't the vision be fixed since the lens isn't going to change over time? I am thankful for my sustained sight, but I can't seem to get past the grief of compromising my distance vision to save my sight, because it has severely limited my ability to function as I used to.

Usually it’s not perfect but it’s still better when you have your surgery but it sounds like it went bad in complicated. It just goes to show you don’t do stuff until you absolutely have no choice.I think you should totally see new doctors and get a second evaluation if cost allows

This doctor is unreal at breaking the info down and presenting it back. You come over extremely well and it’s very educational 🙌🏼 I am meant to be getting multifocals tomorrow in the UK however I am worried about HALO effects. I do a lot of night driving and they are doing both eyes 👀 they always do they tell me. Optical Express I am going with. Long distance is terrible. Mid distance is good. Reading is good but not close up small print like On a jar I massively struggle. I also don’t like glasses makes me anxious wearing them as they slide up and down and because it’s bifocal they move slightly and the vision is out of contrast. I then need laptop glasses as well. So multifocal are meant to be my best option. What do you think Doc? 🙌🏼😇

New subscriber, just got consult scheduled 04/2024 for cataract surgery. I had just gotten new trifocals that I could not adjust too, double vision in right eye predominantly. I’ve worn tri’s for many years, never could get comfortable with progressive lenses. I’m a retired LPN (graduated in 1971 - when patients returned to the unit having to lie flat, & heavy sandbags each side of head). Boy has this surgery changed in the last 50 years! My OD was hoping we could get by before having the cataracts addressed, right is worse than left, but this last Rx change I could not adapt to. Having neurological changes following a horrible case of bacterium sepsis in 2018 has left me with instability in walking & balance issues & the new glasses made things even worse. I’m fascinated by the technology changes in all of medicine, so it should be interesting to see how things will change for me in the next few months. Looks like I’ll be doing some binge watching to get educated prior to that consult. Living in Rochester, NY, home of Bausch & Lomb, seems they have played a role in many of the changes we are seeing today. Thank you for sharing your knowledge and expertise, it is very much appreciated, I immediately pulled out my gel drops and will make sure to utilize them regularly prior to my appt.!

My husband and I both had the 5th option - Multifocal IOL. We each did both eyes at the same time so it was blind leading the blind. All went well. Husband drives for work at night and got used to the stars/halos very quickly (weeks). I took a bit longer to not notice (12 months). My husband's only complaint is he can see a tiny crest of darkness at the outer edge of each eye, whereas that disappeared within weeks for me. My main issue is that daytime glare is more annoying and I also experience some issues reading computer pages. Another complaint is that fluorescent lights make reading and seeing difficult, for example when shopping in grocery store. I have light coloured eyes and suspect that I may be light sensitive compared to some other people. I also sometimes experience annoying blurry spots. Over all we have both said yes that we are glad we did it and would do it again (not that we will ever need to). I can read tiny print without glasses and can see across the table, across the room and out to the horizon very clearly. Equally, I am not going to promote it as I do think that each individual must weigh pros and cons. If your eyesight is very important for your job, example a surgeon, then I would maybe not mess around. It's not perfect but for us it has been positive and successful.

The critical piece of info for people to understand is that any extended range towards near in vision comes at the cost of image quality and contrast. Monofocal has the least range near but absolute best image quality overall. Physics of optics.
@paulabrown52438 ай бұрын
This is true.
@bhavanivishwa66504 ай бұрын
Do you mean monofocal is best to opt for
@Sipu794 ай бұрын
@@bhavanivishwa6650 if you are picky about your vision, then you will get the absolute best image quality and the best dynamic range and color with a monofocal, It's also the easiest and safest. But you will need glasses to see near (or far depending on what kind of lenses you select). One thing is important to also understand that there's no guarantee that the lens calculations will hit their target perfectly, they have +/- 0.5 dioptres of error. Modern lens calculations are very good but you may still miss your mark. My both eyes ended up slightly hyperopic because the doctor used a plano lens. My general vision is very good, but i need correction with glasses for sharp vision. When corrected it's pretty spectacular. With multifocals my vision would have likely been far worse as the doctor has more targets to hit. It's better to be a little bit near sighted than go over as you have sharp vision at some distances, farsightedness means you won't have sharp vision at any distance.

I'm grateful for this video. I recently went to a doctor for my cataract. It was an eye center and I didn't like that it felt like a factory. The doc wants to put me on Xiidra and an antibotic ointment for my blepharitis, she says to get my eye in the best shape for surgery. The Xiidra was $566 and the antibiotic was about $350. I said no, I'm not paying this. The blepharitis can't be cured so I'll still have it after surgery, so what's the point? She didn't even discuss lens types with me, or how the surgery works, or what I can expect after surgery or any need for glassss afterward. I felt like she just wanted pharma kickbacks for the meds. I rescheduled with a solo practice doc who did my parents cataracts. I'll see what he says. But now I kind of want to wait for the newer lenses in 3-5 years. I do have astigmatism but I don't know how bad.
