I see most people haven't yet figured out the concept of AI itself, leave alone using it in organisations. But we'll all be there sooner or later :)

There is a huge amount to learn about AI, because it is not a single thing, but a broad range of technologies. Anyone who thinks it is just ChatGPT and its equivalents is missing most of the picture. But, I agree - the knowledge will seep into business once early adopters start to show successes.

Adoption is certainly at the early stages, especially in Uk vs US and also dependent on company size. However as Mike said Ai is a broad range of technologies. For many companies / employees their first interactions with AI are likely to be in software that is already in place and that has added some form of AI enabled functionality such Adobe or Salesforce etc

What is this related to? I find it hard to accept that we can apportion blame in the abstract. I don't doubt that there have been cases where software engineers have, by neglect, error, or even malintent, have caused harm. But, as a sweeping generalization... this is, frankly, nonsense.
