I keep thinking about this. If we have lets assume 365 effects, and set effect cooldown to 360, then first 360 effects are random as normal. But then, when the game chooses 361 effect, it has only 5 options -- 5 new effects and the first one. If it chooses the first effect, then the pool contains 5 last effects and the second, if one of last effects is chosen then there are 4 effects plus effects 1 and 2 in the pool. This means that after 5 last effects exit the pool, new effects are added in exactly the same order as they were previously, and therefore the order does not seem random. This happens always when the cooldown is close to the amount of effects we have. I am not sure what happens if the cooldown is bigger than the amount of effets we have, as there will be a point when the game does not have any legal effects to choose from, but the only way to increase randomness in this case is to not set cooldown too high, as this way effects will come in an order very closely resembling the previous one

Kingdom Come Deliverance is really nice game. Developers had some talk online and described how during development NPCs were disappearing and nobody knew why. Later they found out they simply died in very random ways. For example fell into hole in forest and couldn't get out, so they couldn't eat and died of starvation. Yes, that game runs NPC code in background even while you are at different place, it's not just easy scripting.
