As some have said, the Doctor has two different personality vectors (maybe more), one of which is the impulsive but firm, mercurial "no second chances" type (Tennant, Tom Baker), and the other more mentally complicated, multiplexed and calculating (Smith, McCoy).

@@atharvadeshpande6907 A bit late to the party, but... See, that's the thing. We know and love the Doctor as this kind man, a man who gives second chances. We love him, because he dances in burning London and yells 'Everybody lives, Rose! Just for once, everybody lives!'. But there always was another Doctor, one who is cold and calculating. Who claims in 'The Waters of Mars' that he is the Master of time and then later on tries to quite brutally enforce this masterhood. He is the one who ends his own race, because he sees that this is the only way to end the Time War. The Doctor was always a man who was manipulating not 'only' time, but his companions, his enemies as well as his allies and even whole cultures in order to achieve his goals. He even declares himself that a good man has no rules, because he doesn't need any and that there are reasons why _he_ has so many. He _tries_ to be a good man, he often is, but just as often he is not. It's not that I like this part of the Doctor, but it is _there._

When he mentions The Doctor visiting the sister once a year, every year, from who's perspective are the years passing for? The Doctor is a time traveler. The sister could go a hundred years thinking "is this the year he forgives me and lets me go." Meanwhile, The Doctor gets a hundred years worth of visits done in a couple hours while the entire ordeal happened just last Tuesday. Or is it the other way around. The Doctor waits a year before visiting each time, but the sister is thinking "why does he keep leaving only to show up a few seconds later?"

The Mother of Blood won't experience eternal life, but the rest of the universe will observe her with it. When you fall into an event horizon, the universe ages infinitely while you experience finite time, so from an outside perspective, you never fall in, nor do you age beyond a certain point, nor do your actions progress in any way beyond that point, nor do you even reach that point. Your maximum age becomes an asymptote.

The galaxy portion makes sense. The gravity of such a thing is so great that it even distorts time and light, thus she can’t even age.

The chains and black hole are just ways to contain them. The Doctor probably has the technology to make people immortal, he just has an issue about it (I'm remembering bits of the episode with Mark Gaitiss). Time Lord's seem to prefer regeneration over immortality; less risk of stagnation.
