Is there not enough difference between a smas and a high smas to have added it into this discussion? I had this procedure 22 years ago along with fat and am just now contemplating having a little refresh with the same surgeon and technique. Also, I've seen some amazing results with the croton oil/phenol peels, if I needed resurfacing I would definitely choose this. Thanks so much for this great discussion!

Could you tell me who is your surgeon? There is definitely a big difference between high smas and smas. High smas is even better than deep plane

Hello, thank you for your videos and for being so instructive and speaking so candidly. I would like to know if what's usually called a mini facelift is the same as a SMAS-ectomy and if that addresses the mid-face area properly and the sagginess over the nasolabial fold as well as oval sagginess. I've been told that the DEEP PLANE Face and Neck Lift definitely does address that area too but my biggest concern, besides nerve injury and other procedure complications, is the release of the face retaining ligaments in the long run. I know it allows to better reposition muscle and skin but ligaments are not reattached through the SMAS and to the skin again, are they? So after 10-20 years, how does that affect the aging process? Ligaments are there for a reason and although they may create hollows and sagging, they still hold the face together. If nothing is holding it to the bone, won't it just melt down altogether? And therefore needing another surgery to reposition them again? I guess my main question is, is there a way to reattach those ligaments through the SMAS and to the skin so you'll have anchors from bone to skin at the places they're supposed to be instead of muscles sitting on top of the fascia and the skin sitting over the muscle and fat with nothing to keep it all together? Great question! It is true that a SMAS lift doesn't release all of the retaining ligaments. There are no ways to reattach these ligaments but there is extensive scarring beneath the skin that acts as an anchor.Depp plane facelifts does release more of these ligaments and from my experience seems to last longer but that is a subjective impression.

There are no ways to reattach these ligaments- I think this has been a misconception from many of the videos describing the release of these ligaments. But yet none of them describe actually re-attaching the ligaments. Yet I could have sworn a Doctor telling me that he would re-attach those.

I have been thinking about going for some type of procedure for quite some time and I have really appreciated the insights of you and your expert associates during my decision-making process. I still haven't quite made up my mind....your thoughts on thread-lifting would be much appreciated. Please keep up the good work!

I don't believe that thread lifting is something I would recommend . it has potential complications for a short benefit which does not make sense to me

Thank you for your video. If a patient has had a DPFL and sagging has returned within the year of surgery and wants a revision, is the revision typically another DP or a SMAS?

It is very interesting to me that after I lost 98 Lbs those hallowing appeared on my face. I never had face lift or any kind of surgery is done on my face, actually, because of those hallows I was thinking to get a face lift...lol

If you lost that amount of weight. You might benefit from fat grafting. Just keep that in mind.

Question: have had 2 s Lift/neck facelifts - 1 at 55 and one at 60. Now I’m about to be 66 and the waddle and creasing/jowling around my lower face and neck are back like before. Do you recommend a Deep plane lift this time? I feel as if the S lift has a short shelf life for me.

I’m currently researching surgeons for a facelift at 55. There’s a surgeon in California that does fat grafting at the same time of the procedure on over 90% of his patients. He’s a very gifted, high end surgeon that has consistent and stunning results. However, I’m just not sold on it. It’s permanent and I just don’t want to be overfilled.

Good thought-not everybody needs fat grafting but some of my best results have been in patients that have had both

What about cheek implants instead of fat grafting? And if a postmenopausal woman is on HRT, would that speed healing?

Cheek implants in the appropriate patient is certainly an option.Every procedure has its pros and cons.

I've learned that most surgeons with this experience combine the DP Facelift and DP Necklift together. It wasn't clear whether either of you 2 Doctors always include this. Also 3 out of 4 consultations I've had this year, have included the Fat Grafting with the DP Face/Neck procedure.

Some patients only need a neck lift and yes commonly a neck lift and facelift are performed on the same day.

Im about to chose procedure with my surgeon. Fat and nano woth deep plane and neck. Im menopause pre menopause. I dont know now what to ask for.😢 I have no face fat as it is but now I'm wondering if this is a good idea. Plus im now concerned about the anchor points. I also had bad aftercare with breast reduction and left bad scaring. So is this a good idea

It is confusing because there are many ways to arrive at excellent outcomes but it depends on who is performing the procedures.

​@drkopelman I just saw this dr. I am advised he will be administering botox to weaken a strong muscle and I'm wondering where he will be weaking. Have tou heard if this as I can't find anything on this

Drs by your reasoning, your first facelift will determine the type of facelift the patient will be able to have going forward (as one ages) as the secondary revision will be too high-risk to go from a SMAS to a DPFL due to the potential disruption of the facial anatomy.........is this an argument for doing the DPFL as the first FL in one's 50s?

Yes ,I think a revision deep plane facelift adds risk because the anatomy is rearranged.

Like many things in life, the proof is in the pudding. Before and after photos tell an important story. However, so do stories of complications and why those complications occurred. Bedside manner is nice to have, but I will take surgical acumen and artistry over bedside manner any day.

UCLA Dr ruined my eyes with fat graft. WE call him Mop Head in the chats due to bad hair piece and so many botched patients he gives out NDAs like candy to keep his intagram fam.
