At 9:56 AM on June 30th, 2011, a truck carrying explosive material's brakes had failed and had blasted through the Girralong railway crossing. The hazardous cargo fell out of the truck, scattered on the road and tracks. At 10:01 AM police arrived and began interrogating the driver on the contents of the cargo. At 10:03 AM it was discovered the material was hazardous and should be cleared off the tracks. The hazmat crew was dispatched from the nearby major city of Capital.
Near 10:07 AM the crew decided to contact the signaller of the station. However, for an unknown reason, word of this incident did not get passed along to the signallers and got lost in the process. At 10:10 AM, a train appraoching Girralong was given the green and hit the explosive material on the tracks. It then derailed and hit another train ahead waiting at a red signal.

Injuries (on the train and ground) : 138
Injuries (on the train) : 134
No casualties were sustained on the ground.
