Susanoo cousing pain seems to be gone once the Eternal Sharingan is unlocked. Sasuke loosing Susanoo wile blind and Madara still able to use it whit no even having eyes probaly means that there a requirement to do so its probaly unlocking the EMS or mastering the Susanoo.

Yeah that's how you unlock what he's calling the complete body susanoo, he is describing the perfect susanoo. The complete body susanoo just gives the humanoid one legs giving it mobility, all the other stuff is added to in the perfect susanoo form. You need EMS for perfect susanoo but not the full body one.

Yeah it’s probably a plot hole like the first guy said, but u CAN make an argument that it only becomes permanently accessible to those who developed it that were actual uchiha, and that kakashi only gained access because of the (bigger plot hole) of Obito, uh, *Kamui-ing out of the afterlife to bolster and support Kakashi* because in kakashi hiden he flat out says he’s struggling to accept the hokage position, because he can’t use any sharingan abilities anymore. So madara and shisui using it, and Obito never using it supports the idea that it’s either an ability u can permanently use only if you’re an uchiha, and requires two eyes to learn, or it’s just a plot inconsistency, and plot convenience that kept Obito from having one, but also allowed madara and shisui to use it without both eyes
