"The more you suffer the more it shows you really care, right?" This line is from the perspective of the person with low self esteem who is wallowing in self pity and the warped logic that the person making them suffer will some day appreciate the suffering and will eventually care about them back because of how much they are making them suffer. Of course this will not happen, but if you have low self esteem it probably makes sense in the moment. Maybe they are asking this question to themselves in their head to justify to themselves why they are letting another person make them suffer.

The part of this song that I'm always a fan of, beyond the "just a sucker with no self esteem", is when he goes "Right?!" about the idea the more you suffer the more it shows you care. Because he doesn't love her, she's a weight around his neck and he knows it. But he's trying to convince himself the suffering is worth it, because it's easier than getting some self esteem and believing he's worth more than this. The word sounds vaguely desperate, because he needs to believe it himself more than he needs you to.
