Hi Doctor, I was prescribed this medication and it worked great for me. I'm now into my 12th week. I have a couple of questions. 1. Can cabtreo be used after 12 weeks? I have a lot of medication left and plan on using it until I am out of it. 2. Does Cabtreo get rid of acne marks? (I have a fair skin)

1. Other than isotretinoin, most acne treatments work best at managing acne, rather than putting it into long-term remission/clearance. Thus, continued use is often important to maintain results. Trials are only 12 weeks long because it is more expensive to do them for longer and one can demonstrate something works typically after 8-12 weeks. 2. Great video! I just got prescribed this medication, how long would you recommend I be on this medication? And how long would it take for antibiotic resistance to form?

Antibiotic resistance generally does not seem to occur with use of benzoyl peroxide (which is contained in this product). In terms of how long for a typical person, most acne treatments (other than isotretinoin) are managing the acne, but not curing it, so they typically need to be continued until the acne is no longer an issue (often gets less with age).

my derm just prescribed me this, but i have sensitive skin and it sounds pretty irritating. would using the sandwich method work with this? also is this supose to help with sebaceous filaments and closed, comedones, or just pimples?

That would be similar to tretinoin or adapalene in effectiveness. So likely about 3x less effective than something like clindamycin-adapalene-benzoyl peroxide.

Most topical products require long-term use since they do not “cure” acne but instead help to manage it. In general, there do not seem to be any harms observed with long-term use.

I recently watched your video where you mentioned the potential benefits of Vitamin D and Vitamin B5 supplementation for acne. I was wondering if you have any opinion on Myo-inositol as a supplement?

Myo-inositol (and spearmint tea) are both thought to be helpful for conditions such as acne due to their potential antiandrogen properties. However, since they are not a prescription medication, I think people often falsely assume that they are completely safe approaches with no risks. While these might be helpful strategies to treat acne, I think we have to consider them similar to any other pharmacologic treatment. Just because they come in the form of a tea or supplement doesn't mean that they carry no risks of side effects, etc.

I didn’t see great results with tretinoin. Do you think this will be a good option? I was just prescribed it to use on face, chest and back. (I have keloids on chest and back but treating the acne). Also, great video just subbed.

Glad that you found this video helpful. This channel is not for medical advice, so I cannot comment on your specific situation. More broadly, in network meta-analyses, clindamycin-adapalene-benzoyl peroxide is much more effective than tretinoin on its own. In general, getting rid of acne often requires combining multiple treatments that work in different ways.

Can this be applied at night before bed? That would make it even more convenient for an athlete that trains in the sun

This product can be used at night, though the benzoyl peroxide component could potentially bleach fabrics (i.e. pillowcases, sheets). However, if the concern is sun sensitivity, it will occur regardless of when one applies the topical retinoid. So sun protection will still be important.

I’ve been using Cabtreo for 4 weeks and I still find it irritating. I wash my face, put dry and then allow my face to dry for 30 minutes before apply Cabtreo. I then apply my moisturizer 10 minutes after the Cabtreo was absorbed in and I’m still experiencing irritation and hour later after apply Cabtreo. Why is that?

I cannot speak to your situation or provide personalized medical advice, but about 3% of participants in the clinical trials had to discontinue Cabtreo due to side effects. Both retinoids (adapalene) and benzoyl peroxide can be irritating and some have difficulty tolerating them or cannot tolerate them. In some of the other videos we go into strategies to avoid irritation from retinoids, but using topical retinoids every other day (works 80% as well) and applying moisturizer first can help some people.

It's a product that is still on-patent and has an optimized vehicle (thus no generic available). The market exclusivity results in higher prices that we see for all on-patent drugs.
