Another missing factor was a LOTO (Lock Out, Tag Out, Test Out) Certificate 😢, which could help the new shift discover the error. The other one was communication between the two Shifts. The day shift would have informed the next shift of the mentainance progress. The other factor is the PTW. A new shift means canceling the first Permit and raising a new one. When you cancel and Raise a new permit, the permit issuer who is usually an HSE Personnel is supposed to do an inspection to ensure that the site is safe for work to proceed before approving the PTW.
