The reason Dr.D wears a mask is during a lab accident; he invented a device that'll make him speak to the undead. Meaning he'll talk 2 his mom in the afterlife,unfortunately it exploded and as a result it ruined Doom's face by giving him a huge scar. So he ventured to Tibet where he encounters some monks and forced them 2 create a suit that'll hide his mug.

The reason is he had a really bad scar in college even worse made scar bigger by fusing his entire face with the mask while it's still hot.
