I did it by slamming the hog into him, completely bypsassing the door

you can kill him three times. toss grenades before the door closes, then park you warthog up against the door. when the door opens in the cutscene the marines in the hog will blow him to bits, then kill him finally when you reach him in the end of the level.

Even back when I played CE on the original Xbox, I never had him come back to life; even when the cutscene plays where he's supposed to come out the door he would just be a corpse on the ground. When I was little I even scummed the checkpoint right before he closes that door so that I could stick him then since I found it difficult to beat him in the corridor later.
