I can only speak for myself, but I rely heavily on a thesaurus and a rhyming dictionary - if the word I'm trying to rhyme doesn't let me do it, I find another from the thesaurus and try to rhyme that instead. It's like rummaging for pieces to fit into a jigsaw. Also, pick a rhyming scheme which 'fits'. I spend a lot of time with that. As for the story, I get a lot of inspiration from fantasy art which often suggests something to me. The audio part? Read out loud - a lot! You do get better!
