hydra feels good in general cause u become an actual champion that can shove and roam or side lane . Maybe it's not the best item to rush as 1st 2nd and maybe you go serrated dirk 1st then tiamat then close 2 items and close hydra

maybe you just have adapted so much that you have forgotten how nice it feels to buy tiamat. being able to actually clear waves is sometimes so valuable! the extra autocancel after hail of blades feels good too.

I think its not the issue of hydra being a bad item, I think the issue is the lethality works differently now hence why hes saying these items will be meta since the value of lethality > wave clear ability. Of course, he cant be 100% accurate until he tries out the items, but I think thats the mentality he has for choosing these items. Profane does give lethality, but as he mentioned it wont be fast enough to snowball since tiamat doesnt give lethality and with the new lethality buffs, the impact that not building lethality is bigger.

I think hydra after the 2 core items and maybe after EoN will be strong. You enter E Q AA AA AA and hydra to get the lower threshold or maybe to get more quicklier there and quit possibilities of healing saving them from your ult. Also in mid game or late game if you need to farm or depush can be useful.

If you fall behind would profane be worth over lethality? Like if you get camped before dirk is even an option and fall really far behind, or just buy dirk still and pray you get a double kill botlane?

You should definately prioritise dirk because its better than tiamat stats wise but tiamat will definately be a good buy after dirk if you need the wave clear especially against champs with good waveclear early. Otherwise a complete hydra is more of a mid-late game item. If u don't need the waveclear it's better to complete ghostblade then buy tiamat especially if ur ahead.
