To fix your problem around 12:00, you can simply extrude and then toggle the option "keep faces together" to off in the popup menu.

For anyone having trouble getting faces like on the bottom of the feet, the end of the hand, and wherever else you want them flat: after you extrude the faces press "R" and click the square thats is on the side you want to flatten and pull it back towards the model. Sometimes the square will be above the faces instead of below and in that case just drag it towards the faces/vertices you want flat. That should flatten the faces and save you from rotating multiple faces and vertices! Afterwards you can rotate all the faces as needed and the will stay flat.
