The way it works is if you're holding a shotgun for example, the Supply Specialist will drop you shells. If you hold only 1 porta bunker in your hand at a time, (not your inventory) it will drop you 3. If you have 4, it will drop you none. It can also occasionally drop utility items that you don't have as well, as seen in the video.
