From the title I wouldn't have realized how *very important this subject could be* to a lot of people. You should consider doing a "Part-2" going over the Who, What, Where, Why, and When of the entire process scenario. From who would need to do this, to why it's important, where to get and submit these forms, when and how to submit and when to expect a response, and who to contact...etc., and any other pitfalls that may arise and how to possibly overcome them.

Would this work if the Father of a minor won't communicate with us at all. He Doesn't want to do anything with his daughter since she was born. We don't even know where he lives he don't give us any information like that and he blocked us in social media. Can we get the minor a passport with all this circumstances? Or do we still have to go to the court?

You typically need two-parent consent for passports and international travel. In my case, however, I won full legal and physical custody of my daughter so I am allowed to get her passports and travel with her without permission from her mom. Idk your situation so it’s hard to say what path would be best for you

It's a strong likelihood and my advice is to include as much supporting documentation as possible. In my case, my divorce decree stated that I have physical/custodial rights with joint custody with his father. I text him multiple times and sometimes he responded and other times he was unresponsive. I did not have his address and he refused to provide it. Finally, I got a lawyer.

I have full sole custody of my child; however, Dept of State is asking for DS-3053 or DS 5525. At first, the father is not responsive, but he eventually responded back that he will only sign the DS-3053 with 2 conditions. Should I include all those details on the DS-5525 form? Are you sure you're using the correct form? DS-5525 is used when you cannot obtain a notarized letter from the other parent and you can't prove they consent to a child's passport...

Yep, just double check all the info is filled out correctly, and if you look at the couple of pages that appear before the actual application, they will inform you what documents/protocols you should follow for your appointment :)
