I have two additions: 1. Thyroid hormones can diffuse into cells passively, as well. However the rate of diffusion wont be enough to provide target functions. 2. If TH receptor did not bind the hormone, it would associate with co-repressors which inhibits the gen transcription further. So, the lack of the hormone will lead to profound effect on target tissue due to both lack of activation and exaggerated inhibition. To exert its effect, the receptor needs to be dissociated from co-repressors and binded to free T3, then the receptor will form heterodimers with RXR to accelarte transcription. Thyroid hormone resistance is due to many reasons, but one of them is loss-of-function mutation of the TH receptor beta gene. In this scenario, those mutant receptors have low affinity, and despite accumulation of the large amount of active hormones, TSH will remain elevated or inappropriately normal, since it does not sense or react the T3. This disorder should be in mind and needs to be distinguished from the patients with TSHoma and dysalbuminemic hyperthyroxinemia.
