The Bullit burnout scene is completely on script. A smoke machine is in the trunk, with holes drilled in the undercarriage, to ensure the desired effect is achieved.

What often happens is that the actor does something unscripted like this, and if the director really likes it, they'll redo the take and adjust the shot accordingly. It's unscripted, not spontaneous. Something spontaneous would be like the Joker's reaction to the hospital bomb not immediately going off in The Dark Knight. That's a pretty famous example because redoing that shot would be prohibitively expensive.
