My volume (unassisted) is really low - and the assisted volume keeps getting lower.

For me I noticed that with Chrome the volume is a little low (I still can hear it well though), but with Firefox it is very high and very good. There is an app for Chrome that lets you raise the KZbin volume even higher, it's called KZbin Volume Controller, you could try that.
