About Mutagen-man, he was then known as Timothy a.k.a. Pulverizer. He volunteered for testing the mutagen. (then turtles came and crash the party but then Pulverizer pored mutagen on himself) He turned into moving ooze. Turtles then put him into giant container to stop him. He was safe until he started slurping mutagen. That's how he got arms and legs. Then to stop him they gave him a special mutagen. (by special i mean the freezing one) He slurped, then he got frozen.

For the rest of the series he was better known as "The Iced Mutagen Man". It could change later on season 4.
