The spell seems like it only works when Toji is in the vicinity of strong sorcerers. The grandma wasn't really "strong," but she technically fit the bill so he had to kill her. I think the "instinct" mode kicks in when he sees a strong sorcerer or curse, hence why he was in fight mode with Dagon and Megumi.

So it was his body winning the tug of war with the Son’s soul. Toji’s soul temporarily came back probably due to the technique the Granny was using, but the moment he offed her, his soul was lost to his body due to the technique becoming imbalanced, and his body started working off instinct, and his instincts have him go fight the strongest. That is why he doesn’t speak until he sees his son, and his soul pushes through for control again. And to make sure he can’t rampage again, he kills himself.

The thing about that technique is that it ends once the body runs out of cursed energy, problem is... @gamerdweebentertainment1616 the pact sukuna made with yuji was made for his own ambitions, not to work together. Sukuna even expressly stated he doesn't care if yuji dies with the fingers. Sukuna is fine the way he's written. Instead of being just a plot device for the mc to get stronger, he becomes a character that viewers r interested in and want to know more about.

I was so disappointed when Toji died again. I was so hype to see more of him, but no, he's too OP, so the plot demands he must die otherwise the arc will last ten seconds.

It wasn't plot. Gege could've easily made toji sense sukuna since sukuna was the strongest and end toji there, or have megumi summon mahoraga. The purpose for toji to die where he did was to keep his character consistent and expand on it. There's no redemption arc to be made for him so him fighting on the sorcerors side would be out of place. Him fighting for tge curses or kenjaku would be out of place because his only beef was against the zenin clan and sorcerors. Having him die against megumi allowed us to know what his intentions were when selling megumi and expanded his character in a good way

If anything, Toji will still be mentioned SEVERAL times as the story goes on, mainly by Gojo cuz Toji really traumatized him.
