The Bone Trader from the Grim Hollow Monster Grimoire is a fey that sucks out bones from targets as it's primary attack. It will now have a gang of tooth fairies.

The pliers attack should also cause disadvantage if a spell has a verbal component. You ever tried talking after getting a tooth pulled?

But that is mostly due to the anaesthetic, and I don't think tooth fairies carry Novocaine.

@@derekstein6193 The anesthetic makes it easier to talk. Part of your spine's function is to reflexively oppose movements that cause intense pain, before those sensory signals can enter the brain and consciousness. Unless one attends extra focus to to what they are doing, their spinal reflex takes over, limiting movement. Of course, a divided focus creates more clumsy movements as well.

@@grilledleeks6514 Are you aware that there are multiple kinds of anesthetics as well as multiple kinds of nerves? Morphine, for instance, blocks pain signals but not other kinds of signals when injected into a nerve.

My headcanon: Bone Fairies grow from a full set of golden teeth and the powder of 10,000 normal teeth, and are the royalty of the common Tooth Fairy because they, on top of supercharging the powers of Tooth Fairies in their immediate vicinity, also allow tooth fairies to grow their own teeth naturally, which also helps keep their population up, since they no longer have to raid human settlements for teeth.

Oh my...so many directions to take this one in! Comedy (tooth fairies that take teeth so large they get weighed down), extra horror (tooth fairy line their pliers' mouths with teeth to trigger tooth-crunching phobias), society (bone-smithing fey, ossuaries as shrines)...there's literally nowhere this can't go.

I was thinking of creating this head sized insect monsters that lay eggs in peoples gums and after a week or so the little bugs will burrow out and use the teeth as shells. Then the bugs use the sharp ends of the teeth to burrow into the victims head and take over its mind making a zombie like creature. Then have the village the party is in be attacked by tooth fairies constantly, and the party will find out that the fairies are actually trying to destroy these bugs to save everyone.

You should look up the disease called river blindness (the one treated with ivermectin) if you want some inspiration. It sounds very close to what you're describing.

I can imagine a low level party having an encounter with an owlbear and all of its teeth have been removed by tooth fairies. What the party expected to be a combat can turn into a social encounter very quickly. Maybe they'll even be able to befriend the owlbear and give it replacement teeth or get its teeth back. Or they just kill it because initiative has been rolled and you need to attack something during your turn.

I totally forgot about that, though curiously, it actually seems they have teeth in their beaks from the illustration in the monster manual and I have a vague memory hearing something like that in a video from AJ. Regardless, the same point still stands for creatures like wolves and normal bears.

I'm glad you've come around to tooth fairies​. I'm actually (indirectly) using them in campaign. I propose toothfairies collect teeth as a business, hence why they pay for teeth under pillows. Anyone can pay for one or multiple teeth from their collection from a given individual, and use them as spell components, such as for scrying, voodoo dolls, or what have you. The fairies collect teeth from anyone since they're cheap (a copper piece each or less) and you never know when some rando farmer's kid becomes a legendary hero. I suppose you could commission a tooth-napping from them as well? Exchanging baby teeth for coins is probably so traditional for most people they don't think about it more than, "Fairies, amiright." Strahd asking my party "Do you believe in the tooth fairy?" is something they never expected lol

I like to imagine that the reason gold teeth are so important to them is because since gold is highly conductive, meaning electricity can easily flow through it, so some create mechs out of teeth and metal

I do remember one time a swarm of Tooth fairy's actually became a primary antagonist in a campaign I was playing. Enough of them actually swarmed a nearby dragon lair and actually stole a few dragon teeth before retreating. But the idea presented was THE tooth fairy was their patron/queen and she wanted to gather the essence of a bunch of creatures (never found out why) but like other Fey can touch someone or use their name to gain power over them, the tooth fairy was able to do this with teeth and began taking control of parts of a kingdom over time. When we eventually confronted her, in a last ditch effort, she actually created a giant tooth golem to fight us, which was essentially like a bone golem if I recall, but not undead and bigger/stronger. It was wild that the campaign took that direction for a bit.

Mystara straight up has a skeleton arch fey called bloody bones. Perfect for these dentist feinds and there dire cousins. He's very evil so keep that in mind if your Using him for a PC patron

The dental aid idea would be unlikely to work; A tooth does not need to be pulled unless it is rotten/damaged/etc, and I doubt they want damaged merchandise.

That isn’t true-I had four teeth pulled for my braces to go on, as they were crowding my mouth-just make them a cosmetic dentist! And wisdom teeth often need removing.

Okay, I had an idea for these. I had this idea for an early game quest (as in level 1) where the PCs are tasked with finding the diary of a necromancer who had been defeated by a party already, so they have to go through the old lair and many traps are still intact but the dangerous enemies are all gone except maybe some weaker undead. I was going to just have the final encounter be a couple skeletons with a third stronger skeleton or maybe a zombie, but now imagine it: the PCs enter the boss chamber, on the ground are 2 or 3 dead Tooth Fairies, 2 more are trying to get at an armored skeleton while he swats at them. The squeaking of the rusty door hinges causes them to pause and look at the party as they enter the room. The two fairies look at each other excitedly and clack their pliers while shouting "Fresh teeth!" in Sylvan and turn to charge the players. Meanwhile the skeleton glares at them and a raspy voice eminates from it, "More intruders?" and raises it's weapons and begins approaching, roll initiative

Mini story time: I was running a ecologist campaign where the players were writing essentially a monster manual from the pov of adventurers. I wanted to take the bone fairy meme and turn it into reality. Sooo that's what I did. In a little village next to a cursed forest late at night do not wonder down the streets, lest you be visited by the bone miser and his little pixie helpers.

Loving the Wu-Tang shirt. Also, I have to wonder how much older I am than you, or how poor my tooth fairy was when I was a kid, because I sure didn't get a Loonie. I got quarters. Elk emblem quarters. Audio is a little funky at the start, but once "Combat" starts the audio is much cleaner. Some of the pics used are absolutely creepy and disturbing. Good lord, dude. lol Honestly can't imagine you finding a monster more horrifying that this little pest. That said, you have "promised" that the next one won't be as bad, so you'll just have to pick one from the list that's not pure nightmare fuel.

This is a terrifying creature. They could strip a town of teeth in a few nights and nobody may know why it's happening. They are fey so they are some of, in my opinion, the most dangerous types of creatures. Fey are malicious and give zero fucks about anyone. Imagine being a parent and going to wake up your kids to find all of their teeth gone and blood all over their pillows. That is pure horror. Now this happens in every house. Every night. For weeks.

I work at a Maxillofacial surgery ward and I kind of hope to one day (when I am a more confident/experienced DM) run a 1-shot at work with some colleagues (especially the surgeons) and the "place where people have had all had their teeth stolen by tooth faries"-scenario is too good not to use if/when I plan for that 1-shot. Great video, great monster, I loved them. I would probably use them as NPCs, they work with the local dentist, the party finds a tooth at a crime scene, the tooth fairy can describe who it belongs to (race, gender, age) but wants a tooth from a monster in return. Usual stuff.

Oh yeah. Yeah i like these guys. Im thinking as an intro to a campaign. The heroes come across a small village on the border of civilization and an ancient forest. Everything is boarded up and there are notices warning people not to be out at night. Tooth fairies have been coming out at night, raiding the town and disappearing off into the woods. They might be just living it up in a cave, or trading the teeth to a hag/greater fey creature. Been working on a germanic/ grimm fairytale kind of camapaign but wasnt quite sure how to get it rolling.

Had a pack attack a party long ago, while the smallest party member was bit trying to hug one. The highlight was a crow form PC ATE one whole on a dive bomb and the rest of the session the PC had tummy issues and rank gas.

Man, this came along my suggestions at the right time. My current campaign has a gnome wizard who is a dentist by profession with a rival who I decided was in league with the tooth faeries. I even thought of mixing in a bone devil for good measure as well, lol... But there's some great ideas in this I can borrow. Thanks!

Hey that’s a great idea. A game I was making for some new players involves a feud between Orcus and the Feywild causing a split between both the abyss and the Fey and both are sort of bleeding out into the same spot on the material plane so I might have a bone fairy setting up near a settlement be what gets the party to find the main plot. Thanks, now I have my main story hook

I don't even play DnD but I'm a big RPG gamer And I love DnD story's even though I have never played and I even watch a funny DnD series from Viva la dirt DnD. But I've recently started watching you videos just to see the monsters and cool areas and I like you style and you have a good head for story telling. i actually made a couple of sessions with "bone fairy". party was lost for days in a fay forest encountering all kinds of bizarre creatures ranging from angler toad to a sparrow with gigantism infamously called "hundred foot shadow". one night, they set up a camp, went to explore surroundings to see if its safe to spend the night. found a pond with small cute white creatures cleaning teeth in the water. This encounter was a bait, since party previously encountered goblins and gathered their teeth for a bounty confirmation. Fairies offered "wishes" for trade.. however our rogue gave them rotten teeth to "maximize" his wish effectiveness. They spent the night in warm blankets, showered in gold, all jolly.. all up until a bone fairy showed up and crashed the party to settle a score with the rogue.

Fun fact: These Tooth Fairies, + monsters like Buggane, Kamaitachi, Delgeth, Alpluachra, Karkadann, Fossegrim, Leanan Sidhe, Cherufe, Scitalis, Bloody Bones, Rawhead, Soulsliver, Harionago and many other mythology/folklore based monsters from pathfinders bestiaries 3, 4, 5 and 6 made it into the books because I wished/asked for them on the forums. Without me, these would not have made it in maybe. They even turned the Fossegrim into a pretty guy and evil, and the Karkadann into a desert unicorn! These tooth fairies Ive seen in Hellboy, and I really wanted them in pathfinder, a simple post on their forum made my wish come true!

The way I interpreted this is that they would absolutely not make a deal with a dentist because then they would only get crappy rotting teeth.

I've been running campaigns in a feywild setting for quite a while now, and a recurring character in them has been a tooth fairy analogue called the Lord of Ivory (inspired by one of the bosses featured in Hit Point Press' Big Bad Booklet), an archfey obsessed with teeth who makes his minions collect teeth for him from the mortal world. Due to a deal struck in ages past, though, these teeth always have to be paid as a way around a part of the deal saying he and his minions can't steal teeth from mortals. He is vain and snooty and all in all a caricature of refined, high and mighty nobility. In a previous campaign, the part had to fight him to get one of the party member's teeth back after a desperate tooth fairy stole them all in his sleep in order to meet her quota. The actual tooth fairy minions of the Lord of Ivory haven't been featured in any combat encounter, though, and I've previously treated them as one would treat commoners. This video and statblock has inspired me to revisit the Lord of Ivory in my current feywild campaign, though. While the Lord of Ivory cannot steal the teeth of humanoids due to a deal, this notably does not include other fey. I can see fey, embroiled in court politics, making deals with the Lord of Ivory to steal the teeth of a rival, marring their beauty which the fey value so much and hindering their ability to use their wit and charisma. The sight of courtiers being suddenly swarmed by tiny fey with pliers in secluded hallways may even be common place when court politics become particularly tense. It could make for a fun and interesting encounter for the party as they gain status in the courts, and inevitably step on a few toes to do so, and / or a plot hook as an ally's teeth are stolen, potentially before an important speech or banquet that the party needs to go well.

I was inspired by this video to build a character (hopefully a PC but at least an NPC) that's essentially someone cursed to be a tooth fairy. Using the Dhampir lineage from Van Richten's and leaning heavily into Shadow Monk and Arcane Trickster.

A friend and me once started writing out lore for a tooth fairy race where they propagate through burying teeth after a ritual. The more powerful the being the tooth came from, the stronger the newborn fairy is. If they are born from the tooth of something like a dragon or the tarasque, they start off as tall and powerful as your average adventurer.

the tooth fairies flutter around stealing teeth and you're like "this is inconvenient but ultimately not that harmful" and then the bone fairy comes along and removes your femur.

I had my favorite encounter ever in Pathfinder with Tooth Fairies. Led by a Devil Imp disguised as a raven wearing a crown of teeth who called herself "The Queen of Molars" a band of Tooth Fairies tricked children into kidnapping animals and other children off the streets to feed their ever growing legions of Tooth Fairies. Of course, there is a tid-bit where it had to be specifically the teeth of children buried in the ground that spawned them... But they didn't know that. So as they collected more and more animals, the they got progressively more frustrated as only a few of the teeth made more fairies. They killed the Tooth Fairies, vanquished The Queen of Molars, and as I started the works of the Queen's return and revenge plot... A catastrophic fight amongst the party halted any progress and we haven't plated since. I never got to do the orc raid I prepared months for, the demon summoning old man I was waiting for them to stumble across, to reveal the friendly wizard was actually the son of a now lich and was working with the orcs to help them raise an undead army for the Devil Masmu (where there was a whole thing with him pretending to be a demon and making the orcs sign away their servitude to him in return for power), or the catacombs I made at the start of the campaign that lay under the city waiting to be explored. I could use those again but they have been tainted by the memories and so they will probably sit on my shelf ad infinitum... Or until I get a new group since I don't have a table to DM.

I'm quire a bit late to this video(Just found the channel, sorry!) But I 100% ran a short 3 session game where I had the party solve a mystery about villagers randomly loosing teeth and eventually found out that their bones were being stolen! I wished I had your stat block at the time; I just respected fairiees! Lol

One of my homebrew monsters was called the Blueberry Fairy. She would cast spells on people that transformed their teeth into blueberries, collect the berries, and then flee.

When I think of Tooth Fairies, I honestly think of the ones in Hellboy 2. Yeah, cute at first, until they start chomping down on you, going for your teeth first. Never ever underestimate Fey creatures, they will trick, terrorize, hurt and possibly even kill you especially if you laugh at them and or look at them wrong. Everyone laughs, until a tooth fairy yanks one of your front teeth out with a big pair of magic pliers.

Tooth fairies are usually good and do help remove bad tooth from people. A witch needed a lot of tooth as a spell component or for a rite to summon an aberration or some shit, so she made a pact with a/multiple Tooth Fairies to gather tooth for her. This can be a plothook for the campaign you're running, or it can become their backstory by saying that this pact corrupted the fairies and created a new sort of Tooth Fairies.

I like to imagine that the way that they propagate is like this. Once they steal a tooth they deposit a piece of their flesh onto the root of that tooth. That flesh then begins to grow from around that tooth like a weird mold. Therefore each tooth fairy starts their life with one tooth.

The tooth fairy is a perfect example of something Spoony said a while back “If you ever want to get the players to go somewhere, steal something from them it doesn’t have to be important you just need to take something that is theirs” I’d even wager you wouldn’t even need to give them the disadvantage, they will hunt those fairies to the ends of the earth to get their teeth back.

For me, tooth fairies are the unwilling (yet reverent) subjects of the Unseelie Court’s self proclaimed artist: Marromhaim. Lord of macabre fascinations, father to the outcast and friend to the shunned. Good ol’ ‘Haim takes pride in seeing beauty where others can’t and loves nature’s freak. As the story goes, some sprites once snuck into the Artist’s Bone Gardens, skulking past the flesh-sculptures and human trees to the object of their obsession. Polished to a pearl gleam they just couldn’t help themselves. Nor could they save themselves from him. Never the most vengeful of his court, he saw a poetic use for them; branding them with their own desire to steal teeth and demanding a tithe from that day on.

A good plot hook would be the adventurers overhearing a mother warning her child that if he’s bad, the tooth fairy will come and pull out all his teeth. they think it’s just a fun story and flavor but…

In pathfinder 2e, the toothfairy is now a swarm. I just fought one, and I think it was like CR 2. oh, and it definitely doesn't 'just' try to pull out your teeth now. Now they are big fans of biting you to death as well, hellboy style.

I remember it being said somewhere that fey put a lot of stock in "perceived value." It isn't just "you ate an apple of a tree I own, now give me your first born" it's "that apple I gave you was the most DELICIOUS apple from my FAVORITE tree so I want something you value just as much, if not more!" Well what if they take the teeth because of their *perceived* value to the mortals they've recently been stolen from? One tooth, in reality, doesn't make any difference to how well one chews but in that scary moment you realize it's gone, it will sure *feel* like one of the most important things in the world to get back ASAP. You're also on the clock to get it back before it's too late. Thus putting you on the backfoot of fey bargains.

I'm a filthy little Pathfinder player, but they have these buggers in the game as well, along with a MONARCH version of it that leads them, is as large as a human child, and carries a staff and crown made of teeth. The Monarch can also cast fumbletongue at will which is pretty funny. Oh, also, instead of making you sleep the Pathfinder tooth fairy actually can bite and paralyze you. Fun!

Imagine a city that starts to reverencing the tooth fairies, they attract adventures with quests and when they sleep in the inn the villagers open the doors so that the tooth fairies can go in and steal the teeth

The party is hired by a local noble to catch a local swarm of these only for the party to find out the noble is a vampire and they just got their fangs back. Or they run into a tooth fairy that stole some elephant tusk and is using them. Well that’s terrifying. Actually could make the tooth fairy much higher CR threats by having them steal teeth from many other creatures and using their biting attacks and effects.

God there's a low level dungeon of these things in the Pathfinder module Tyrants Grasp. Giving me flashbacks here to how badly they traumatized my character, and giving me evil DM inspiration to now use them on my 5e group that have never touched pathfinder

If your party falls asleep in enemy territory, aka in a dungeon, make them do a perception roll if they take a short or long rest. Easy nightmare fuel even on upper level parties.
