why totally different structure for "can I have the bill" and "can I have water?" Is it possible to use both ways? e.g. palihug ko og tubig, or pwede ko mangayo sa bill?

Yes. Just like the English language we can have multiple ways to say it. And it also depends which is the focus. For pwede ko mangayo sa bill? The "sa" act as the definite "the"
