Hi, I realized this video was uploaded in 2020. Based on the 2023 edition of USMLE Step 1 First Aid it seems wavy fibres appear in the first 0-4 hours and early coagulative necrosis happens in the first 4-24hours. Just thought to mention. Great video! :)

Hey Doc, Love love your videos and it has helped me study for NBME and ultimately STEP 1. Just wanted to make one recommendation and not sure if others have mentioned it before, would it be possible to have the video of you on the left side of the screen. It blocks the questions and I've seen it in some videos. Thank you so much for the time you put into making these videos. It's life changing both as a student and the future generation of doctors!

@@hyguruprep For future vids, if you are using ppt slides, maybe you can edit the slide so that the question is on the left side and the topic, for example: “heat stroke” is on the right side. Also, Thank you so much for these really well integrated videos.

Thanks so much, Matt, for your feedback - I really appreciate it. In building the NBME Top Concepts, I strive to be relevant: www.reddit.com/r/step1/comments/g3dyuy/100_questions_that_appear_on_every_nbme/ and deliver the content in an active recall, integrative, and engaging way. When I introduce a topic, I usually start with an NBME style question. I then break concepts down into: 1. Flow chart or processes for mechanisms of disease. 2. Compare and contrast venn diagrams 3. Synthesized table In reflecting on this particular lecture, the integration part (i.e. where I take one may be using the above outline & veering off for 1-2 minutes) may be contributing to the "no clear structure" or "this guy is horrible" feedback which you provided; I completely understand where you are coming from, however I would love to delve deeper into your specific concerns/feedback. It would really help me on the path to develop as an educator. Hope studying is going well, Matt.

Hi! The vasodilation at the level of the pre-capillary arteriole decreases the stretch at the carotid baroreceptor; as a result, the efferent response to the heart is going to be primarily sympathetic with increased HR and contractility!
