Yep. I've seen it with geriatric patients as well. Some are sharp as a wip and others are not doing well at all. You can tell and see the difference and you can also see their decline from one hospitalization to the next one. It's a very sad thing to see what's happening to them and the grief of their families. Knowing that you have to be very understanding with your patient, that they can't help it and giving them time or to stop expecting them to be able to answer or make logical decisions about much of anything anymore.
