I think the implication of the fat Reimu comment is that Reimu is just universally poor no matter what part of the multiverse you encounter her in, so the fattest version of her is just marginally more fatter than every other version

My guess is fat reimu was a pc-98 reimu who tried to get the power to eat sweets without gaining weight only it turned out the power was a lie. Still being reimu she lost the weight through the poverty and tea diet but now she has the weight of an average girl instead of a featherweight one.

That's one aspect that's unlikely to improve. With their new helper system they arguably made it even worse (the system itself is good, especially for new players, but it's implemented quite clumsily). But the stories are interesting, the artworks are cute, and the devs finally did a "fine, I'll do it myself" on ZUN by reviving both Shinki and Mima.
