I strongly advocate for mitral valve repair over the use of the Mitral clip, as surgical repair offers a more functional and physiologically effective restoration of mitral valve function.

EVEREST II shows that a percutaneous first option is safe, with less adverse events than upfront surgical option for primary MR. Worst case, surgery down the road in about 20% of patients treated with clip. For Secondary MR, there is 0 data to support surgery at all, where as MitraClip is a historically beneficial intervention.
