I have been bed bound for 8 months due to multiple blood clots in both legs.I was in hospital 1st time for 3 days with leg swelling back pain down both legs. Never did ultrasound on my legs on CAT scans. Had UTI - Kidney Infection- and sepsis. After 3 days discharged with legs still horribly swollen was told to follow up with pain management! Went home within 3 days back to ER they did ultrasound on legs guess what blood clots and my recalled IVC filter was clogged and had moved. Thrombectomy was done but the radiologists stopped because I was throwing to many clots. Had baseball size hematoma around hip groin area started bleeding out in icu after procedure. Ended up after 3 weeks in hospital going to nursing home with very limited physical therapy (a few steps everyday since their were still clots in my legs) a doctor there told me it could be months before I walk and would probably be limited. Lost 90 lbs in 21 days at nursing home to sick to eat. It was horrid. Went home Nov 2018 been in bed ever since. Legs are still swollen with constant never goes away pain. Home health care could only send 1 person so that wasnt going to work trying to get me up. I cant get to my reg physician ambulances just dont come to your home here in Ohio to take you for any follow up care. Ive been back to the ER 3 times but since there are no "new" clots just the old ones I get sent home with nothing. They say lymphedema. Only time they did the correct cat scan to check IVC filter was when I spoke up. They don't like or belive i guess in the ivc filter issue. I have been on warfarin 1st from a saddle PE size of a dime in 2011 and multiple clots in my lungs 7 months after that. Just 2 years ago I was put on Eliquis so I wish someone would tell me how I ended up like this on Eliquis with this many blood clots and bedridden. Therevis a place where I live called Kindred Hospital which should have been offered to me by Kettering Hospital back in October. They are inpatient therapy but now I have to 1) Get family physician to sign off for me to be admiited (but I cant get to them for them to see me or 2) Go to ER be admitted and stay for 3 nights in order for Medicare to approve! Im 59 years old one day in Sept 2018 at work got up couldnt walk to my car. Ended up lising my job going on SSDI which just about pays the bills. The Eliquis Im on at beginning of 2019 went to 472.00 for 30 day supply. Then after that pay Humana 30.00 a month and 47.00 @month for Eliquis. I got Eliquis in Jan for a 1x free thing. So now I haven't had it for 4 months now. Did buy some chaep aspirin. ER doctors let you go back home like that. Either Kettering Hospital ir Medicare will kill me. Sorry so long but Im tired of how Ive been treated but they think you're a problem whiner or bitch if you talk back to them.

Can pharmomechanical thromboectomy be applied to upper extremity DVT. Acute day 7 and on Xarelto
@DrSudi5 жыл бұрын
Yes it can. We tend to do this if the DVT extends all the way up the arm and into the chest. It really depends on how extensive the DVT is.

Thank you for a quick reply. The UEDVT is in the sublavian/axilliary vein. Thought to be Premarin induced + Paget-Schroeder Syndrome. Although not young (age 60, female); but 3 days before presentation had done elliptical twice for 40 min, swam 50 laps, yoga, shovel snow. Obviously stopped the Premarin.

My DVT clot(s) are four months old. I have had multiple dopplers. Wondering if stents are always used or is mechanical thrombectomy sometimes enough? Thank-you.
@DrSudi Жыл бұрын
Stents are NOT always used. They can only be used in the iliac veins which are the deep veins in the pelvis. Stents cannot be used in the veins down the leg. In arteries, stents can be used down the leg but not veins. Often mechanical thrombectomy is enough but that is really for fresh clot. If the clot is old (greater than 1 month), it likely has become scar tissue. This scar tissue can be broken up with balloon angioplasty in a procedure known as deep vein recanalization (DVR). Read my article below on this topic. www.gethealthyveins.com/2021/12/05/deep-vein-recanalization-for-the-treatment-of-post-thrombotic-syndrome/

Hi sir..what will be procedure performed if thrombus is detected more than 14 days of its formation? Catheter guided thrombolysis? Thrombectomy? How many percent of thrombus can be removed if it of 5-6 cm
@DrSudi2 жыл бұрын
After 14 days, catheter directed thrombolysis can still be done - depending on the specific case. Single session thrombectomy can also be done in some cases. It's hard to say how much thrombus will be removed after 14 days as each patient is different. Some have already developed more chronic thrombus than others after a couple weeks. The key is to see an experienced vascular specialist asap after being diagnosed with DVT. Remember that not every DVT needs a procedure. Many can be treated with anticoagulation alone. In general, the higher up the leg the DVT goes, the more likely a procedure may be warranted. DVT that extends into the pelvis should definitely be evaluated by a vascular specialist for possible thrombectomy.
