Trifurcation Left Main PCI in a NSTEMI patient using Multiple Bifurcation Techniques with ‘Ping-Pong’ - Prof. Dr. Fazila-Tun-Nesa Malik
MBBS, FCPS (Med.), MRCP(UK), FRCP (Edin), FACC, FSCAI
Chief Consultant Cardiologist,
National Heart Foundation Hospital & Research Institute,
Dhaka, Bangladesh
CASE SUMMARY:
In this case we applied multiple bifurcation stenting techniques:
LM to LAD & LCX DK crush
LCX & Ramus TAP
LAD & Ramus Mini crush
Dual route femoral and radial used.
Catheters of either route were engaged and disengaged as required (Ping Pong technique).
TAKE HOME MESSAGES:
Tackling Trifurcation Left main Lesion using Multiple Bifurcation Techniques may be an option in a situation where CABG can not be performed.
The ‘Ping-Pong’ technique enabled intervention in complex trifurcation Left Main lesion.
IVUS should be considered for all Left Main and complex PCI procedures to optimize stent deployment.
Optimal Left Main PCI has significant impact on patients long term outcomes.
