Is it required to use the bottom level if you have a big mattress? what if I want to use the top level so the bed can sit up higher?

Is your floor level? Not all floors are created equally. If it's not level, maybe you can use furniture levelers. There are tutorials for that on KZbin. Also, do you have support beam? If it didn't come with the bed, see if Ikea sells one.
