UDID Card Apply Online Form Kaise Bhare|How to Apply Online for UDID | Help 4 Special

The UDID project initiated by Department of Empowerment of Persons with Disabilities aims at building a holistic end-to-end integrated system for Issuance of Universal ID & Disability Certificates for Person with Disabilities with their identification and disability details.
The UDID card shall bring a host of benefits to the Persons with Disabilities as given below: Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader.
UDID Card will be acceptable everywhere to prove one's disability condition. “At present the disability certificates of one state are not recognized in the other.

In an attempt to create a National Database for PwDs, Department of Empowerment of Persons with Disabilities has been issuing a Unique Disability Identity Card (UDID) to each person with disabilities. ... UDID certificate is not mandatory for availing any direct benefits for PwDs whereas Disability Certificate is.
Person with Disability will click on register link to register with UDID Web Portal.
Using credentials PwD logs in to system and click “Apply online for Disability Certificate. ...
Upload color passport photo and other requisite documents like Income Proof, Identity Proof and SC/ST/OBC proof as required.

Thank you very much, your information really helped me. I have already applied in district welfare office and received the certificate, how to link it with ud id? Edit: in Tamil Nadu, every Tuesday District Disability Welfare camps are conducted in every District Collectorate where doctors assess the disability level, how ever it is beneficial to share disability assesment docs done from any hospital (govt or private) to make our work easy.

Hi no need to send hard copy ,its only needed at the time of assessment, or regarding delay in appointment u can directly contact on given help line no on website, they will guide u everything
