UDID card 1st time bana Raha hu. Nair hospital se Dr. Ne writen me percentage ka certificate diya hey, Online Form submit ke liye Dr. Ne diye hua certificate ke details online form me add karne hote kya ? Aur details submit karke certificate aur registration no. Leke card ke liye return hospital Jake registration no Dena hota hey kya ?
