In fact, it's simple. They deleted all the files related to Sheeva but they forgot to update the list of characters loaded in the random selection. When you press Up+Start the game selects only the characters from Mortal Kombat 3.

on the genesis version Sheeva is way more well hidden, if you try to load her by any way the game instantly crashes.

no olvidaron borrarla, sucede que los personajes comparten codigo y es practicmente imposible eliminar por completo un personaje sin romper los otros, lo que si eliminaron fueron los sprites pero solo los necesarios para incluir a los personajes nuevo, segun escribio hace muchos años un usuario de un foro, el codigo para seleccionar personaje aleatorio en modo torneo recorre todos los indices de los peleadores excluyendo los jefes y puesto k sheeva no es un jefe olvidaron poner un candado para que el algoritmo se saltara a sheeva en caso de caer en ese indice

O legal é que às vezes com esse bug da sheeva é possível trocar a música do stage do roof que está com a do subway pela própria original do roof é até uma versão meia misturada das duas mas tem que jogar sempre no roof

You can't, at least through regular means. My guess is that when you reach the end of the game (if you somehow manage to not crash it) with her you'll either get her MK3 ending, a black screen, a garbled mess or just a crash. Maybe someone should try that...

It was just to make room on the limited cartridge space (they took out the announcer voice, plenty of stages, animalities, and Sheeva). She was in the regular version of MK3 on the SNES.
