I've had a transplant because of PBC 28 years this September. I could stop the itch by wrapping cold damp towels on my feet and holding damp cold cloth in my hands. Had the rest of my body covered to stay warm. It worked and I could sleep, as you said nights were the worst. I still take urso everyday and anti rejection meds.
