nice video, but with this one-half rule, most implants would need bone grafting to satisfy the 2B rule. That means more cost and morbidity for the patient. while this rule considers the MD width of the tooth it does not consider the BL width of the bone.

Great observation! You’re absolutely right. The 1/2 rule is the starting point, and the implant diameter is then adjusted to meet the 2B rule. If both criteria result in a diameter that is mechanically insufficient, only then would grafting be necessary-minimizing the incidence and cost of unnecessary bone grafts.

Maybe I missed it but where did the data for the charts of implant diameter (success rate x mm) and implant length (success rate x mm) come from?

Hi Doc, great question! In the section where those charts pop up, I'm simply using an example scenario to demonstrate the principle of parametric and global maxima. They're simply example numbers!

Where would we get the data to determine which length is most successful in each position? Or is that unknown?

@@salival7246 The short answer is it's not possible to get definitive data for every case and patient, as each case is unique and needs a custom solution. However, with the 2mm rule (choose an implant that's as long as possible without coming within 2mm of vital structures), it's not really necessary to have that data anyway!

I have great respect for the efforts of the Doctor to place the video. I hv five implants in the mandible and six at top with pfm dentures. When i chew food i get a mild pain on one implant at mandible.All others are excellent.

In the virtual surgery you plan for an idea crown and free gingival margin. It is completely independent of the existing bone contours. Typically you use the adjacent teeth to determine ideal esthetics and function.
