I know why the eye does this! There are several nooks and crannies on all the maps where Fade eyes and Yoru flashes get stuck. These spots exist where world colliders are just barely separated enough to allow some util to slip through, but go untouched for the most part. It happens to me a ton because I main both agents. Hope this helps some of you!
