Thank You , dr.Madaan , for such an informative and useful video . Agreed with content, except that step 2ck s 60% is step 1 content. Maybe it was that way years ago, nowadays and with step 1 pass/fail , it’s totally different beast by itself. It’s so much and devil in details.Step 1 , with meticulous and precise information, seems easier to prepare compare to step 2ck exam.Step 2ck retain maybe 20-30% of step 1 information, but it does not make step 1 less important. Agreed that step 1 pass/fail made everything ( study for step 2ck, matching, extras) much more complicated.:(

@@Ramakrishna-Dash Because usually IMG's are clinical physicians who practice medicine in their home country. So Step 2 CK is easier than Step 1.... I studied 7 years of medicine in Chile, because in this country you are obliged to become a general medicine physician in order to graduate. Step 2 CK questions are easier than step 1, for me at least.

I am a 4th year MBBS student in Pakistan who is considering USMLE. The thing is,I study in an insane way where I go through the book and verbally repeat the important points or points to remember 100 times. It takes super long like 6 literal hours for a chapter,but it works like magic where I can retain 80-90% of the information forever. At the end of 4th year,I would've covered all the material related to step 1,I can just lightly and quickly review all the material and it would be as if I am fresh on. Do I have much to worry about . I am shocked to hear that for step 1,people choose pathoma for pathology when I am out here memorising Basis by heart for Pathology. In my own batch,I got 3rd place in 3rd year and I am also able to retain microbiology very well. I am scared of the step 1 exam because of the different style of studying that people choose to be able to pass the exam. Can you please guide me on this? Like retaining the information is zero issue for me. Pathoma looks horrendous to me for the literal lack of information it has. If I choose to do step 1,I would most probably do it mid of 5th year,so I would have around a year and a half to prepare. Also,how important are connections? How hard would it for me to get used to step 1 questions and basically excel at this considering that I've learned anatomy,pathology, etc. At a deeper level than what is found in First Aid.

I understand the importance of basic sciences for medicine, but the truth is, that when you are making clinical decisions you are not thinking about krebs cycles or ß-oxidation pathways. So shifting the focus to Step 2 CK shows better the clinical application of knowledge... I've had classmates who used to rock biochemistry, physiology and pharmacology, who are still in medical school, because they don't have what it takes to pass the clinical rotations. In my opinion Critical Clinical Thinking is much more harder that the basic critical thinking of basic sciences, however that doesn't mean that basic sciences are not important, it just means that it is not a huge necessity for all specialities. The only one that really needs a good solid base of basic sciences are internal medicine, genetic, anesthetics and pathology.

I think this is beneficial because step 1 pass/fail encourages students to give step 1 in 3rd year mbbs. Once they have pass report. They can do US electives directly by extending internship (as electives are given for undergraduates only) and then seriously prepare for Step 2 as if they are giving step 1 first and step 2 continuously as before the change. And then have good step 2 scores and then continue their internship in India.

I agree. This is so ridiculous to make medicine become preschoolish and hand out pats on everyone's backs. Look there should be more emphasis on mental health, counselors and even more time to study for the step 1. BUT, you can't just make it so it's a plus grade for everyone and then expect the physician pool to be strong. The expectation of even the best candidates academically has fallen and so even they slack off. I worry about the patients now.

@@SunSunSunn No NPs and PAs are still not up to par knowledge-wise. I would never use one for a family member though there might be some great ones the vast majority are questionable. I would also avoid FM, sadly. The reason why they are more attractive is they are more accessible and have little ego involved so patients appreciate them better. I personally think MDs/MBBSs have huge egos that get in the way of their decisions at times. Many cannot apologize or admit their wrongs. Many dislike second opinions or their decisions questioned. I think that is ridiculous because I personally would want others to catch my mistakes so would listen to everyone but most MDs are petty. All medicine as a system should be reviewed. There needs to be classes for humility for everyone and also anonymous feedback by real patients as to their care. The person being given the feedback should not know which feedback comes from which patient. Also, patients need to be able to speak to someone in an accessible manner if they or their family members are not getting the questions they are looking for answered or they have doubts about the care being given. A mere survey weeks later does not cut it; neither does having to escalate to some stranger in some far-off place. It should be immediate. right there in the hospital. There should be a liaison or go-between for each patient, who is advocating for the patient. They should not be hired by the hospital but by a third party who gets compensation from the hospital. So it costs the hospital money when they hire bad medical staff. And the go-between has a neutral ground to work. They don't answer to the hospital but to the third party. That third party is there to review the hospital and perhaps give out ratings for consumers. Med staff should be on their toes because of this.

Hello i have posted this before on your another video but i really need help. So I hope you can see it and reply. I need some information. I'm a 2nd year med student in bangladesh. I want to go to the USA after completing my MBBS here. But including everything, just the total cost of USMLE will cost me around 30-32 lakh/3-3.2 million BDT (30k-32k USD). And not to mention the cost of living in america. I need to be prepared with at least 40-45 lakh/4-4.2 million BDT (40k-42k USD). My parents will never let me go abroad. Firstly, they don't have that much financial ability to send me. And I myself hate it if i need to depend on my parents money. I already get to hear so many taunting for getting into a private medical college with their money. I don't want to be the loser in my parents eye anymore. Secondly, my parents are extremely restricting and toxic when it comes to every single step of my life. I've been going through so much depression for so many years due to them and society. I really need to get out of here. And it has always been my dream to study abroad. But it's not possible for me to earn 40-45 lakhs even if i try to earn money for 4 years through some part time jobs while studying and start working as a doctor after my study is complete. If I can't get out of here on my own, i will be forcefully married off by my parents. My life, my wish don't matter to them. They only care about what they want me to do and what achievement will make them more proud in society. My dream and whole life will be crushed. I really need to earn money or a scholarship for it. But is there any scholarship for medical students for studying abroad? Or is there any scholarship to bear the cost of the exam only at least? If there isn't any such option, what can i do? How can i earn? Or is there no way other than just leaving my life and dream and accepting the fate my parents will choose?

that depends. on one side some programs feel its extremely handy and shows commitment, while on the other hand some might not like it as you are an old grad (not so fresh with your medical knowledge) and you might not like taking orders from people younger than you

How does it make someone with an MD from another country appear as someone with not so fresh medical knowledge? That person has been in touch with medicine (whichever branch it is) and in particular clinicals and clinical exposure right.

Sir, I hope to pursue medicine in Georgia 🇬🇪 and they say the program is 6 years long including the internship and its been my dream to work in USA .so what do u recommend is it better doing the internship or no need of doing it ( does it affect when finding a residency as I have no experience ( as no internship done) please reply need your help sir to get a decision please sir reply

If Step 1 scoring was adding so much pressure to students.... then why not just keep it scored and just not give it so much weight while selecting candidates for good residencies? Why not just select more Step 1 240s for plastic surgery? That would lower pressure yet still give students an extra opportunity to show a good score to help their application.

Brother I am giving neet ug this year.u are my role model.i passed 12 in 2020.in my first drop,I scored 453.when I started studying really hard,I started to get above 650 in every mock tests of allen.but 3 months b4 exam this year,my mother was diagnosed with cancer. I couldn't appear in neet 2022 bcz I felt standing beside her is more important than giving exam.i started studying again to fulfill my dream since my mother is cancer free now.bhaiya after seeing u,I decided that I will also go for usmle after mbbs.but in some places,I am seeing that Indians can't appear for usmle after 2023.is it true?will my dream always remain a dream?

A lot of the hate on step1 pass/fail move sounds like it’s stemming from the old generation being butt hurt that all their insanely hard work and mental suffering they endured during their step 1 studies is know meaningless and the new doctors won’t have to bear the heavy burden of memorizing huge amounts of almost totally useless empty jargon. Step 2 teaches you what you actually need to know and going to use as a doctor and you can ask any practicing physician at any point of time whether they used Kerbs cycle or most of the other nonsense they were unwisely forced to imprint on their souls. I so much hate step 1 and forever happy that it wont torment the young no longer. The application process will be a bit harder for a while especially for imgs but in the long run people will see the blessing of the non heavily mentally scarred and better clinically competent doctors thanks to step 2
